脉压对脑梗死患者颈总动脉影响的超声观察

摘    要:目的:使用超声技术观察脉压对脑梗死患者颈总动脉的不同类型斑块的发生情况和物理形状的影响。方法:用二维高频超声和M型超声观察A组187例脑梗死患者颈总动脉的不同类型斑块的发生情况、内中膜厚度、收缩期管径、舒张期管径,结合血压,计算颈总动脉的动脉硬化指数、动脉可扩张度和周向扩张率,并与B组232例非脑梗死者比较。结果:脑梗死组和对照组及各分组之间比较,脑梗死组中PP≥60mmHg组与血压正常组比较、PP<60mmHg组与血压正常组比较,斑块的检出率增加,中内膜增厚,内径增加,动脉硬化指数增加,动脉可扩张度下降,周向扩张率明显下降。脑梗死组中PP≥60mmHg组与血压正常组比较,这种变化更明显。脑梗死组中内膜普遍达到有病理学意义的增厚(大于1mm)。结论:高脉压型高血压在脑梗死的发病机制中有重要的作用。如果超声发现高脉压的高血压患者颈总动脉中内膜增厚,检出斑块,内径明显增加,动脉硬化指数明显增加,动脉可扩张度明显下降,周向扩张率明显下降,则高度提示脑梗死的可能,从而为脑梗死的防治提供帮助。

Abstract:Objective:To observe the relationship between pulse pressure and the frequency of each type of plaque and physics charater parameters of common carotid artery of patients with cerebral infarction using ultrasound.Methods:B-mode high freqency ultrasound and M-mode ultrasound examination was performed on group A: 187 cases patients with cerebral infarction to measure the frequency of each type of plaque,intimal medial thickness,systolic diameter,diastolic diameter.Some physics charater parameters such as arterial sclerosis index, arterial expanding degree and dissepiment peripheral expanding rate were calculated by ultrasonic measurement with systolic pressure and diastolic pressure of brachial artery.Comparing with group B patients with no cerebral infarction.Results:Patients in group A (A1,A2,A3) had higher FTP,higher ASI,lower AED ,lower DPER than patients in group B(B1,B2,B3). Patients in group A3 had more higher FTP (P<0.01 ),more thicker IMT,more higher ASI,more lower AED,more lower DPER than patients in group A1. Patients with cerebral infarction have ordinary thicker IMT.Conclusion:High pulse pressure has a very important role in patients with cerebral infarction. Ultrasound could observe the relationship between PP and FTP and PCP of common carotid artery of patients with cerebral infarction.It is important for preventing and treatmenting the cerebral infarction.
