66例子宫内膜癌预后相关因素分析

目的：总结分析子宫内膜癌的预后指标，以期有助于筛选高危病例，指导治疗。方法：对66例子宫内膜癌的临床病理参数，利用COX回归模型进行预后分析。结果：在单因素分析中发现患者的年龄、组织学分级、肌层浸润、宫颈侵犯、盆腔或腹主动脉旁淋巴结转移、腹腔细胞学阳性、和非内膜样腺癌类型(透明细胞癌和浆液性乳头状囊腺癌)与预后显著相关。在多因素分析中，组织学分级、肌层浸润和宫颈侵犯与预后相关。腺鳞癌、腺棘皮癌与无鳞状上皮化生的内膜样腺癌的预后无显著性差异。结论：组织学分级、肌层浸润和宫颈受累是子宫内膜癌的独立预后指标，有助于评价预后。

Analysis of prognosis in 66 endometrial carcinoma

Objective:To investigate the prognostic factors in endometrial carcinoma. Methods:A total of 66 specimens of endometrial carcinoma were analysis by COX regression model. Results:Age, histological grade, cervical invade, muscle invade, pelvic or para-aortic lymph node metastases, peritoneal malignant cell, clear cell and uterine papillary serous carcinoma type were significantly associated with prognosis in monofactorial analysis. In polyfactorial analysis,histological grade, cervical invade and muscle invade had significant relation to prognosis. Prognosis of adenocarcinoma with benign or malignant squamous differentiation was no different from endometrioid type. Conclusion:Histological grade, cervical invade and muscle invade could be used as independent prognostic factors for survival in endometrial carcinoma.
