摘    要:目的探讨p16在睑板腺癌(meibomian gland carcinoma,MGCA)中的表达及其生物学意义。方法采用免疫组织化学方法(SP法),检测p16蛋白在65例石蜡包埋的睑板腺癌标本中的表达,并在不同分化程度及生物学行为的睑板腺癌之间进行比较。结果65例睑板腺癌组织中53例表达p16蛋白,30例癌旁组织中29例表达p16蛋白,两者差异有统计学意义(χ2=10.548,P=0.014);睑板腺癌中,p16蛋白表达阳性率随组织学分级降低而降低,不同分化组中p16蛋白表达差异有显著性意义(χ2=15.000,P=0.001)。不同生物学行为睑板腺癌中p16蛋白表达差异显著,无复发组p16蛋白表达率显著高于复发转移组(χ2=24.890,P=0.000)。结论睑板腺癌中p16蛋白表达异常提示其在睑板腺癌发生发展中起着重要作用。p16蛋白的表达缺失或失活是睑板腺癌发生的重要分子事件,并与睑板腺癌的分化程度、浸润能力及复发转移密切相关,可以为临床判断预后提供依据。

Abstract:Objective To investigate the expression and the biological significance of p16 protein in meibomian gland carcinoma (MGCA). Methods The expressions of p16 protein in conventional paraffin sections from 65 MGCA cases were detected with SP immunohistochemical method. And the differences of expressions were compared among MGCA cases with different differentiated grade and other different biological behavior. Results Significant differences of p16 protein expres-sion were found between MGCA and paratumor normal tissue (χ<'2>=10.548,P=0.014). The positive rates of p16 protein ex-pression decreased with the decrease of histological grades, thai in the well-differentiated group was higher than the poorly differentiated group (χ<'2>=15.000,P=0.001). p16 expression rates were different among MGCA cases with different biological behaviors, no recurrence group was significantly higher than the recurrence/metastasis group (χ<'2>=24.890,P=0.000). Conclu-sion Abnormal expressions of p16 in MGCA may play an important role in the occurrence and development of MGCA. The absence or inactivation of p16 expression is an important molecule event in the occurrence of MGCA. And it maybe related with the differentiation level, invasion ability, recurrence and metastasis of MGCA. Thus it can be a judgment for clinical prognosis.
