某院抗菌药物临床应用横断面调查

目的了解本院门诊、住院患者抗菌药物应用现状。方法采用横断面调查方法，对本院2011年9月15日0：00～24：00时抗菌药物使用情况进行分析。结果门诊抗菌药物横断面使用率为26．58％，儿科（60．00％）使用率最高，次为呼吸内科（50．00％）、急诊科（30．95％）和妇产科（30．68％）。住院部抗菌药物横断面使用率47．31％，五官科（70．59％）使用率最高，次为外二科（69．49％）、外一科（66．29％）和妇产科（63．24％）。门诊使用抗菌药物8大类28种，住院部使用抗菌药物6大类21种。全院病原学平均送检率为22．22％。结论本院抗菌药物横断面使用水平基本合理，但存在个别科室使用率偏高、围手术期预防性使用抗菌药物给药时机不当、用药时间过长、全院治疗性用药病原学送检率偏低等不合理情况，需要进一步加强抗菌药物合理应用的管理。

Cross-sectional Survey of Antibiotics Application in A Hospital

Objective To investigate the antibiotics application in outpatient and inpatient departments of the hospital. Methods Cross-sectional study was used to analyze the antibiotics application of all outpatients and inpatients in September 15th, 2011. Results The cross-sectional application rate of antibiotics was 26. 58% in the outpatient department. The pediatrics department had the highest application rate, accounting for 60. 00%, followed by the department of respiratory medicine （50. 00% ）, emergency department （30. 95% ） and obstetrics and gynecology department （30.68%）. In the department of inpatient, the cross-sectional application rate of antibiotics was 47.31%. The top 4 were ophthalmology and otorhinolaryngology department （ 70.59% ）, surgical department Ⅱ （ 69.49% ）, surgical department Ⅰ （66.29%）, and obstetrics and genecology department （63.24%）. 8 categories （ including 28 species） antibiotics in outpatient department and 6 categories （including 21 species） in inpatient department were used. Average delivery rate of etiological specimensEUll in the whole hospital was 22.22%. Conclusion In the hospital the cross-sectional utilization rate of antibiotics is basically reasonable. But the application rate in some individual departments is on the high side; the time of prophylactic application of antibiotics in the perioperative period is unreasonable; Medication time is too long, and delivery rate of etiological specimens for therapeutic medication is on the low side. The management of rational application of antibiotics needs to be strengthened.
