摘    要:目的 针刺对硬膜外麻醉阻滞范围的影响再探讨。方法 观察三种麻醉对硬膜外阻滞范围影响。共施行胆囊切除术166例,其中针刺复合小剂量硬膜外麻醉胆囊切除组(A组)56例,穴位皮肤电极刺激复合小剂量硬膜外麻醉和囊切除组(B组)54例,单纯小剂量硬膜外麻醉胆囊切除组(C组)56例。硬膜外麻醉用药种类,浓度(1.5%利多卡因)。给药程序及辅助用药相同,对各组硬膜外麻醉用药后所阻滞麻醉范围进行对比分析。结果 A,B两组减少硬膜外麻醉用药量(1.5%利多卡因)32.96%,31.41%(约1/3);A,B两组硬膜外腔注入相同局麻药量(1.5%利多卡因9ml)阻滞神经节段宽2-3个。结论:针刺或穴位皮肤电极复合硬膜外麻醉可使阻滞神经节段增宽,局麻药量减少。

Abstract:Objective The influence of acupuncture on blocking ranges of epidural anesthesia was discussed again. Methods Three epidural anesthesia methods were employed and compared. One hundred and sixty six patients undergoing cholecystectomy were randomized into three groups: group A in which 56 cases were anesthetized by acupuncture and a small dose of epidural anesthetic, group B in which 54 cases were anesthetized by subcutaneous acupoint electrostimulation and a small dose of epidural anesthetic and group C in which 56 cases were anesthetized by simple epidural anesthetic. Results In groups A and B, the anesthetic administrated reduced by 32.96% and 31.41% respectively; and the anesthesia blocked 2 or 3 nerve segments. Conclusion Acupuncture and subcutaneous acupoint electrostimulation can broaden blocking ranges and reduce the dosage of anesthetics.
