脐带异常859例临床观察与分析

目的　探讨脐带异常的发生原因、常见类型及其对新生儿及分娩方式的影响。方法　回顾性统计 2 0 3 4例新生儿中 85 9例脐带异常的有关临床资料 ,对常见类型脐带绕颈及其对胎头衔接、产程进展、新生儿Apger评分及分娩方式等的影响进行观察 ,并与对照组比较。结果　脐带异常者 42 .2 3 % ,其中以脐带绕颈最多 (3 7.10 % ) ,并以绕颈 1周者居多占 (83 .72 % ) ,提示脐带绕颈与胎儿窘迫有显著相关 (P <0 .0 1)。结论　脐带绕颈仍是影响围产期儿的重要因素 ,强调及时产前检查、早期诊断 ,及时采取相应措施 ,可以降低其对胎儿的不利影响。

Clinical Observation and Analysis of 859 Cases with Abnormality of Umbilical Cord

Objective To investigate the cause and main type of umbilical cord abnormality, and it's effect on the neonates and delivery mode. Methods The c linical data of 859 neonates with umbilical cord abnormality was analyzed retros pectively. The main type of umbilical cord around neck (UCAN) and it's effect on engagement of fetal head, progress of labor, score of Apger and delivery mode w ere investigated. Results UCAN (37.1%) is the most common ty pe of umbilical co rd abnormality. 83.72% of UCAN was only one circle. The result indicated that UC AN was associated with fetal distress (P<0.01). Conclusions UCAN is the most i mportant factor which effect neonates' health. Perfect antenatal examination, ea rlier diagnosis and correct treatment are benefit to neonates with UCAN.
