颈动脉海绵窦瘘的血管内治疗

为探索，改进血管内治疗复杂性颈动脉海绵瘘的栓塞技术和方法，总结了我科血管内治疗１４例的经验体会。

Treatment of carotid cavernous fistula by intravascular occlusion

For improving the occlusive techniques and exploring new occlusive methods of intravascular intervention of complex carotid cavernous fistula(CCF),we reported our experience of 14 cases with CCF treated by intravascular intervention.In these 14 cases,9 cases with simple internal CCF were embolized with ballon;4 cases with mixed internal and external CCF were treated as follows:2 cases with internal CCF were treated by ballon occlusion,the other 2 cases with small fistula of the dural branch of internal carotid artery were untreated;the external carotid fistula of all 4 cases were occluded with solid granula or silk lines;and one case of simple external carotid fistula was occluded with IBCA(isobutlyl-2-cyanoacrylate).The results showed that 13 cases were cured without any complication,one case significantly ameliorated,and all of them had a good recovery.The internal carotid artery remained patent in about 45.5% of these cases.It is suggested that:(1)The effect of ballon occlusion treatment for the internal CCF is good.(2)The fistulas which are extremely small or large are the main causes for occlusion of internal carotid artery.It is currently accepted that such cases can be occluded with microcoil.(3)External carotid fistula generally consists of several minimal branch fistula,which is occluded with solid granula or IBCA,will have a good result.
