MR磁敏感加权成像(SWI)在脑部疾病中的应用

摘    要:目的:探讨磁敏感加权成像(susceptibility weighted i maging,SWI)在脑部疾病中的临床应用价值。方法:收集疑有血管病变的脑部疾病患者37例,其中海绵状血管瘤8例,脑梗塞10例,脑肿瘤15例,静脉畸形2例,静脉窦血栓2例,行常规T1WI、T2WI、DWI、SWI序列及增强T1WI、MRA。评价SWI序列较其他序列对显示小的出血灶、小的静脉及含铁血黄素、钙化等顺磁性物质的优越性。结果:SWI在海绵状血管瘤,能鉴别出血与血管,发现更多的小出血灶。在静脉畸形,能够发现更多的细小的静脉向大静脉引流。在急性脑梗死,SWI可发现小的出血灶,在脑肿瘤,SWI显示出小的引流静脉。在静脉窦血栓,SWI可以清楚地显示深部引流静脉及脑表面扩张的静脉网。结论:SWI是显示低流量血管畸形、小的静脉结构、多发细小的出血以及铁钙沉积十分敏感的新型脉冲序列,可以做为MRI常规序列的重要补充,应用于中枢神经系统疾病的诊断和鉴别诊断。

Abstract:Objective:To study the clinical value of susceptibility weighted imaging(SWI)in brain diseases.Method:Thirty seven cases with blood vessel change of brain diseases were collected and analyzed,including 8 cases of angioma cavernosum,10 of cerebral infarction,15 of brain tumor,2 of vein abnormal and 2 of venous sinus thrombus.Conventional T1WI,T2WI,DWI,the SWI and enhanced T1WI,MRA images were examined to compare superiority of the SWI images in demonstrating the small hemorrhage stove,the small vein,hemosiderin,calcification and other paramagnetic material.Results:SWI can distinguish the hemorrhage from the blood vessel and discover the smaller hemorrhage focus of the angioma cavernosum.It can discover more tiny vein adjacent to the dilate draining vein.SWI can detected smaller hemorrhage in acute cerebral infarction and draining veins in brain tumor.And draining veins in deep part and expanded venous network in the surfaces of brains can be obviously denonstrated when sinus venosus thrombus occurs.Conclusion:SWI is a new pulse sequence that can demonstrate the abnormal low current capacity blood vessels,the structure of small vein,the tiny hemorrhage as well as ferrum and calcium deposits.So it can be applied in the diagnosis and antidiastole of the central nervous system diseases as an important supplement to the MRI conventional sequences.
