社区获得性肺炎210例临床分析

目的 探讨社区获得性肺炎患者的临床特征,指导CAP诊治。方法 分析笔者所在医院210例CAP患者的临床表现、X线表现、微生物学检查及治疗情况。结果 住院CAP患者以老年人(≥60岁)多见,肺炎的临床表现最主要的症状仍为咳嗽、咳痰、发热,但与年轻人相比,老年人易于出现呼吸困难和神志障碍,X线主要表现为片状影或间质渗出;痰培养可明确病原菌指导治疗;痰培养结果出来之前,经验治疗效果尚可。结论 社区获得性肺炎是一种呼吸内科常见病、多发病,临床诊治相对容易。但在部分老年人中症状重,且不典型,伴有基础疾病时可出现预后不良,需引起重视。

Clinical analysis of community acquired pneumonia in 210 patients

Objective To explore the community acquired pneumonia(CAP)in patients with the clinical features,and guide CAP diagnosis and treatment.Methods To Analysis of our hospital 210 cases of CAP patients with clinical manifestations,X ray manifestations,microbiology examination and treatment.Results Hospitalized patients with CAP were most aged people(≥ 60 years).The pneumonia’s clinical manifestations of the most main symptoms were cough,sputum,fever,but compared with young adults the elderly was prone to breathing difficulties and conscious disturbance.X ray mainly showed patchy shadow or interstitial exudative.Sputum culture could make clear pathogens and guide therapy.The experience therapy was acceptable before the sputum culture results come out.Conclusion The community acquired pneumonia was a common and frequently-occurring disease in department of respiratory medicine,clinical diagnosis was relatively easy.But in some aged people with severe,uncharacteristic symptoms and complication disease can occur a poor prognosis,this needed to pay much attention.
