毒瘾者感染性心内膜炎的临床特征分析

为探讨毒瘾患者感染性心内膜炎的临床特征，本文对毒瘾组（８例）和非毒瘾组（２１例）感染性心内膜炎的临床表现和超声特征进行了对比分析。结果显示，毒瘾组金黄色葡萄球菌为主要致病菌，其阳性检出率（６２５％）较非毒瘾组（１９％）明显增高（Ｐ＜００１）；毒瘾组发热体温３９７±０４°Ｃ，较非毒瘾组３８５±０７°Ｃ高（Ｐ＜００５），且多伴寒颤、肌肉酸痛；肺栓塞发生率毒瘾组（６２５％，５／８）高于非毒瘾组（４７％，１／２１）（Ｐ＜００１），而外周栓塞发生率毒瘾组（１２５％，１／８）低于非毒瘾组（４２９％，９／２１）（Ｐ＜００１）；超声发现毒瘾组心腔赘生物检出６例（７５０％），均位于右侧心腔，而非毒瘾组检出１５例（７１４％），主要位于左侧心腔，毒瘾组赘生物较大，最大长度２２±０８（１２～３４）ｃｍ，非毒瘾组只有０９±０３（０５～１５）ｃｍ（Ｐ＜００１）。

Clinical Analysis of Infective Endocarditis in Addicts of Narcotic Drugs

In order to study the clinical characteristics of infective endocarditis (IE) that related with use of narcotic drug,8 addicts with IE comparing with 21 nonaddicts with IE were analysed The primary results were as follows:Staphylococcus aureus was found to be the main pathogenic bacteria in addict group,with the positive rate of blood culture 62.5% which was significantly higher than that of the nonaddict group(19%)( P <0 01) The fever in addict group was more severe than nonaddict group(39 7±0 4°C vs 38 5±0 7°C, P <0 05),and was usually accompanied with shiver and muscle ache Pulmonary embolism occurred in 5 addict cases (62 5%), while only one in nonaddict group (4 7%)( P <0 01).Peripheral embolism appeared in 9 nonaddict cases (42 9%) but only one in the addict group(12 5%)( P <0 05) Endocardiac vegetations were detected by echocardiography in 6 addict cases (75 0%),all of them came from the right heart.15 cases of vegetation were found in nonaddict group(71 4%).13 cases of which were from the leff heart
