中老年糖尿病合并脑血管病临床特点分析

目的探讨糖尿病(DM)合并脑血管病(CVD)的影响因素及临床特点。方法对76例中老年糖尿病合并CVD与同期住院的492例单纯糖尿病患者进行对照分析。结果糖尿病合并CVD发病率随年龄增大、糖尿病病程延长而升高,糖尿病合并CVD组空腹血糖、餐后2小时血糖、糖化血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白水平及高血压发病率明显高于对照组(P<0.05或0.01),高密度脂蛋白胆固醇明显低于对照组(P<0.05)。糖尿病合并CVD以脑梗死多见,病死率高,预后差。结论严格控制血糖、血脂、血压是预防糖尿病并发脑血管病的重要措施。

Clinical characteristics of cerebrovascular disease in the middle age and elderly diabetic patients

Objective To investigate clinical characteristics and the influential factors related to diabetes mellitus (DM) combined with cerebrovascular disease (CVD). Methods The clinical data of 76 patients with diabetes mellitus complicated by cerebrovascular disease and 492 patients with DM were analyzed retrospectively. Results The incidence rate of CVD in diabetic patients increased with the age and course of DM.The concentrations of fasting blood sugar, postprandial blood sugar 2hours, HbA, total cholesterol (TC), triglyceride (TG), low-densithlipoprotein (LDL) and the morbidity of hypertension in CVD group were higher and high-density lipoprotein (HDL) was lower significantly, as compared with those in DM group. Cerebral infarctions often occurred when DM was complicated by CVD and its mortality was higher and prognosis was bad. Conclusion It is quite important to control blood sugar, blood pressure and blood lipid for preventing diabetes mellitus complicated by cerebrovascular disease.
