CRP+1059G〉C基因多态性与冠心病相关性的临床荟萃分析

摘    要:目的评价CRP(C反应蛋白)+1059G〉C基因多态性与冠状动脉粥样硬化性心脏病相关性。方法全面检索关于CRP多态性与冠心病关系的原始研究,将符合人选标准的研究纳入。采用Revman4.2统计软件包及stata10.0统计软件包进行统计,计算合并的效应指标,基因多态性以OR及95%可信区间为效应指标。结果入选的8个冠心病与CRP多态性关系的原始研究采用Mantel—Haenszel固定效应模型合并的(GC+CC)基因型/GG基因型OR值为1.18,95%CI为1.01~1.39,P〈0.05。按地域分层分析固定效应模型中国人群OR为0.85,95%CI为0.64~1.14,P〉0.05欧美人群随机效应模型其0R为1.35,95%CI为1.11~1.63,P〈0.05。结论CRP+1059G〉C基因多态性与欧美人群冠心病发病相关,但可能与中国人群冠心病发病无关,C等位基因携带者患冠心病的危险性有增高趋势。

Meta analysis on correlation of polymorphism of C-reactive protein gene+1059G/C and the risk of coronary heart disease(CHD)
Abstract:Objective To investigate the correlation between the polymorphism of C - reactive protein gene + 1059G/C and the risk of coronary heart disease (CHD) with Meta analysis method. Methods Data bases were comprehensively searched, together with references retracing method, for all the related studies on the correlation between C - reactive protein gene + 1059G/C polymorphism and the risk for CHD. All the related studies were further strictly selected according to the inclusion criteria. C - reactive protein gene GC and C C genotype vs GG genotype OR (odds ratio) and its 95% CI (confidence interval ) were analyzed as an outcome measurement. The statistical packages Statal0. 0 and Reman 4. 2 were applied for investigating heterogeneity among individual studies and summarizing overall effects across studies by proper mathematical model. Results 9 original studies were entered the final data combination. With the Mantel - Haenszel fix effect mathematical model, the pooled odds ratio fork GC + CC)/ GG in CHD group against control group was 1.18,95% CI( 1.01 - 1.39) ,P 〈 0. 05. Hierarchies analysis according to the regional distribution showed the OR of the Chinese population is 0. 85,95% CI 0. 64 - 1.14, P 〉 0. 05, using the random effects model analysis the European and the United States' population, OR 1.35,95% CI 1.11 - 1.63 ,P 〈0.05. Conclusion The polymorphism of C -reactive protein gene + 1059G/C is associated with coronary heart disease in European and the United States population, but there is no association between the polymorphism of C - reactive protein gene + 1059G/C and coronary heart disease in Chinese population, people who carry the C allele ( GC and CC genotypes) have a higher risk to be subject to coronary heart disease.
