剖宫产术后再次妊娠经阴试产115例临床观察

目的 探讨剖宫产术后再次妊娠经阴试产的临床效果.方法 将本院2009年1月～2013年1月收治的115例剖宫产术后再次妊娠患者作为观察组,选取同期本院住院分娩的无剖宫产手术史且行阴道分娩的60例产妇为对照组,比较两组产妇阴道分娩的成功率和母婴结局、阴道分娩产妇第一产程时间及产后出血量.结果 观察组115例产妇中,48例直接行剖宫产,经阴道试产成功47例（70.1％）.A组的新生儿窒息、子宫破裂、产后出血及剖宫产发生率,差异无统计学意义（P＞0.05）.观察组与对照组阴道分娩产妇的第一产程时间及产后出血量比较,差异无统计学意义（P＞0.05）.结论 对于剖宫产术后再次妊娠的产妇,应进行严格的阴道试产指征的筛选,然后进行严密的产程监测,阴道试产是安全可行.

Clinical observation of cesarean section of pregnancy by vaginal trial production in 115 cases

Objective To explore the clinical effect of cesarean section of pregnancy by vaginal trial production. Methods 115 cases with cesarean section again pregnancy patients were selected as observation group in our hospital from January 2010 to January 2013,at the same time,60 cases with hospital birth without a history of cesarean section surgery and vaginal delivery women were selected as the control group,the success rate of maternal vaginal delivery and maternal and infant outcomes,and vaginal delivery women for the first time and labor,postpartum blood loss in two groups were compared. Results Among 115 eases of maternal, 48 cases were directly implements the cesarean delivery in the observation group,47 cases （70.1%） vaginal trial production were successful.The neonatal asphyxia,uterine rup- ture,postpartum hemorrhage and cesarean delivery rate in group A,compared with the the control group,there was no statistically significant difference （P〉0.05）.Observation group and control group of vaginal delivery women for the first time labor and postpartum blood loss,there was no statistically significant difference （P〉0.05）. Conclusion The cesarean section of maternal pregnancy again,to carry on the strict screening of vaginal trial production indications,and closely monitor labor,the vaginal trial production safety is feasible.
