急性自血病患者LDH及SF水平测定的临床意义

摘    要:目的探讨急性白血病患者的乳酸脱氢酶(LDH)、血清铁蛋白(SF)的表达水平及意义。方法采用酶反应速率法、放射免疫法检测118例初治急性白血病患者(87例急性髓系白血病患者、31例急性淋巴细胞白血病患者)、93例经化疗诱导完全缓解的急性白血病患者、37例经诱导化疗完全缓解后1年内复发的急性白血病患者和60例健康志愿者的LDH、SF表达水平,并进行各组间比较分析。结果急性白血病组、急性淋巴细胞白血病组、急性髓系白血病组LDH、SF表达水平与正常对照组差异均有统计学意义(P〈0.05),急性淋巴细胞白血病组LDH、SF表达水平与急性髓系白血病组差异均有统计学意义(P〈0.05)。完全缓解组LDH、SF表达水平与急性白血病组/初治前组、缓解后复发组比较,差异有统计学意义(P〈0.05)。缓解后复发组与急性白血病组/初治前组、正常对照组与完全缓解组LDH、SF表达水平比较,差异无统计学意义(P〉0.05)。结论LDH及SF可作为急性白血病患者观察病情发展、预后预测的指标。

Clinical significance of LDH and SF levels in patients with acute leukemia

Abstract:Objective To investigate the clinical significance of lactate dehydrogenase(LDH) and serum ferritin (SF)expression levels in acute leukemia. Methods The enzyme reaction rate method and radioimmunoassay were used to detect the LDH and SF expression levels of 118 acute leukemia patients with initial treatment (87 acute myeloid leukemia patients and 31 acute lymphoblastic leukemia patients), 93 acute leukemia patients with completely remis- sion after chemotherapy induction, 37 acute leukemia patients who relapsed within one year after complete remission due to chemotherapy induction, and 60 healthy volunteers. The differences between each group were compared. Re- sults The LDH and SF expression levels in acute leukemia group, acute lymphoblastic leukemia group, acute myeloid leukemia group compared with that in normal control group, the difference was significant (P〈0.05), LDH and SF expression levels in acute lymphoblastic leukemia and acute myeloid leukemia group compared, the difference was sig- nificant(P〈0.05).The LDH and SF expression levels of complete remission group compared with that of acute leukemia group/pre-initial treatment group, and relapse after remission group, the differences were significant (P〈0.05). And There was no statistical differences between relapse after remission group and acute leukemia group/pre-initial treatment group, normal control group and complete remission group in LDH and SF expression levels (P〉0.05). Conclusion LDH and SF can be used as indicators for disease progression observation and prognosis predicting for patients with acute leukemia.
