胸锁乳突肌瓣对腮腺良性肿瘤切除术后Frey综合征预防的疗效观察

目的 探讨胸锁乳突肌瓣对腮腺良性肿瘤切除术后预防Frey综合征的临床疗效.方法 将138例腮腺良性肿瘤患者随机分为对照组和实验组,对照组患者行常规腮腺瘤手术;实验组则应用胸锁乳突肌瓣充填腮腺手术凹陷区域;术后随访3～12个月.结果 实验组手术区域凹陷畸形的改善效果优于对照组（P＜0.01）;对照组Frey综合征得发生率为33.33％;实验组Frey综合征的发生率为9.88％,实验组在减少Frey综合征发生方面强于对照组（P＜0.01）.结论 胸锁乳突肌瓣不仅可以有效改善腮腺良性肿瘤术后面部凹陷畸形,同时对预防Frey综合征的发生也有很好的疗效.

Clinical observation of sternocleidomastoid muscle flap for operation on parotid benign tumor in the prevention of Frey syndrome

Objective To investigate the clinical observation of sternocleidomastoid muscle flap for operation on parotid benign tumor in the prevention of Frey syndrome. Methods 138 eases with parotid benign tumors were ran- domly divided into control group and experimental group.The patients in control group were treated with traditional parotidectomy,while the patients in experimental group were treated with modified parotidectomy including repairing the soft tissue defects by sternocleidomastoid muscle flap.Patients were followed for 3-12 months. Results The Frey syndrome occurs rate was 33.33% in the control group,the occurs rate was 9.88% in the experimental group.There were significant lower rates on sunken deformities and Frey syndrome in experimental groups than that of control group （P〈0.01）.The effects of sternoeleidomastoid muscle flap combined with parotid fascia flap were better. Conclusion The sternocleidomastoid muscle flap transfer can obviously improve post-operative deformity and has a good effect on pre- vent the incidence of Frey syndrome.
