摘    要:目的 探讨经后路腰椎椎体间融合术(PLIF)治疗腰椎间盘突出症的临床效果.方法 选取2009年1月~2012年1月本院收治的腰椎间盘突出症患者30例,采用PLIF治疗.记录患者的手术时间、术中出血量、术后引流量及住院时间;术后随访1年,比较术前、术后6个月及1年的视觉模拟评分(VAS)、日本骨科学会腰痛评分系统(JOA)评分和Oswestry功能障碍指数(ODI),评估疗效及植骨融合率.结果 本组80例患者手术时间平均(120±43) min,术中出血量平均(250±80) ml,术后引流量平均(58±10) ml,术后住院时间平均(12±3)d.术后6个月及1年的VAS评分及ODI指数较术前降低(P<0.05),JOA评分较术前增高(P<0.05).随访1年后优良率为90.0%,腰椎间植骨融合率为96.7%.结论 PLIF治疗腰椎间盘突出症效果满意,安全性高.

Effect observation of posterior lumbar interbody fusion in treatment of lumbar disc herniation

Abstract:Objective To investigate the clinical effect of posterior lumbar interbody fusion (PLIF) in the treatment of lumbar disc herniation. Methods 30 patients with lumbar disc herniation in our hospital from January 2009 to January 2012 were selected,all were given PLIF. The operative time,blood loss,postoperative drainage and hospitalization time was recorded;the preoperative and postoperative 6 months and 1 year of the visual analogue scale (VAS),pain score Japanese Orthopaedic Association system (JOA) score and the Oswestry disability index (ODI) was compared,and the efficacy and fusion rate was observed. Results The mean operative time was (120±43) min,mean blood loss was (250± 80) ml,the average postoperative drainage was (58±10) ml,mean postoperative hospital stay was (12±3) d. After 6 months and 1 year,VAS score and ODI scores was lower than that before operation respectively (P〈0.05),JOA score was higher than that before operation (P〈0.05). After 1 year,the excellent rate was 90.0%,lumbar interbody fusion was 96.7%. Conclusion The efficacy of PLIF in the treatment of lumbar disc herniation is satisfactory,and safety is high.
