多西他赛联合奥沙利铂、氟尿嘧啶治疗晚期胃癌的临床分析

目的 评价多西他赛（DOCE）、奥沙利铂（L-OHP）、氟尿嘧啶（FU）联合方案对晚期胃癌的治疗效果和毒副反应.方法 32例晚期胃癌患者均行锁骨下深静脉穿刺或外周肘正中静脉置管,FU由静脉输液泵泵入.化疗方案：DOCE 75 mg/m2,静脉滴注1h,第1天;L-OHP 130 mg/m2,静脉滴注2h,第1天;FU 750 mg/m2,持续静脉泵入120 h,第1～5天.28 d为1个周期,至少应用2个周期进行疗效评估.结果 32例患者中,完全缓解1例,部分缓解14例,稳定8例,进展9例,总有效率为46.9％,疾病总控制率为69.7％.肿瘤中位进展时间6.2个月,中位生存时间11.1个月,1年生存率为40.6％.主要毒性反应包括白细胞减少、胃肠道反应、神经毒性和脱发.结论 DOF方案疗效显著、毒副反应可耐受,可以作为晚期胃癌的有效化疗方案.

Clinical efficacy of the advanced gastric cancer treated with the regimen of docetaxel,oxaliplatin and fluorouracil

Objective To assess the efficacy and toxicity of DOF regimen consisting of docetaxel,oxaliplatin and fluorouracil for advanced gastric cancer. Methods 32 cases of patients with advanced gastric cancer were deep lines of subclavian vein puncture or peripheral elbow in the middle of venipuncture,FU by intravenous infusion pump pumping. Chemotherapy regimens：docetaxel and oxaliplatin were given intravenously at a dose of 75 mg/m2 and 130 mg/m2 for 1 h and 2 h on dl,respectively,and FU was administered as continuous intravenous infusion for 120 h at a dose of 750 mg/m2 on dl-d5.This regimen was repeated every 28 d.The efficacy and toxicity were evaluated after completion of 2 cycles at least. Results Of the 32 patients,1 case achieved complete response,14 cases achieved a partial response,8 cases had stable disease,and 9 cases showed progressive disease.The overall response rate of this series was 46.9%,and overall disease control rate was 69.7%.The median time to progression was 6.2 months,and median overall survival was 11.1 months.The 1 year survival rate was 40.6%.The major toxicities were leukoeytopenia,nausea/vomiting, peripheral neuritis and alopecia. Conclusion The DOF regimen of docetaxel,oxaliplatin and fluorouracil is effective and tolerable for advanced gastric cancer,and can be an good choice for advanced gastric cancer.
