持续呼吸道正压治疗新生儿呼吸衰竭的临床观察

目的研究持续呼吸道正压治疗新生儿呼吸衰竭的抢救成功率及血气改善。方法回顾性分析本院188例出现呼吸衰竭的新生儿急诊抢救资料，根据患儿抢救措施不同，分为治疗组与对照组，治疗组94例行持续呼吸道正压治疗，对照组94例行经面罩吸氧及药物辅助治疗，观察两组患儿抢救过程中的生命体征、抢救结束后的血气分析及成功率。结果治疗后，两组患儿的PaO5、PaCO5均改善（P〈0．05），且治疗组改善较对照组明显（P〈0．05）。治疗组总有效率为96．81％，对照组为89．36％，差异有统计学意义（x2=11．03，P〈0．05）。治疗组呼吸和血压探制理想，住院时间短于对照组，住院费用少于对照组，差异有统计学意义（P〈0．05）。结论持续呼吸道正压治疗新生儿呼吸衰竭安全性好，效价比高，可减少患儿治疗的痛苦，家属满意。

Clinical observation of continuous positive airway pressure in the treatment of neonatal respiratory failure

Objective To study the success rate of rescue and improvement of blood gas by continuous positive airway pressure （CPAP） in the treatment of neonatal respiratory failure.Methods Clinical data from 188 neonates with respiratory failure rescued on emergency in our hospital were retrospectively analyzed.According to different first-aid measures,they were evenly divided into treatment group with CPAP and control group with mask oxygen inhalation and medical auxiliary treatment.The vital signs in process of rescue,blood gas analysis after emergency treatment,and success rate in both groups were observed.Results After treatment,the PaO2 and PaCO2 of two groups both improved compared to before treatment （P〈0.05）,and the treatment group improved more than that of the control group （P〈0.05）.The total effective rate in the treatment group reached up to 96.81%,was higher than 89.36% in the control group,with sta- tistical difference （x2=l 1.03,P〈0.05）.The respiration and blood pressure was more stable in treatment group,hospital- ization was shorter and the hospital costs was less than that of the control group respectively,the difference was signifi- cant （P〈0.05）.Conclusion In the treatment of neonatal respiratory failure,the CPAP with a good safety and cost-effect ratio can alleviate neonates＇ suffering and reflects satisfaction from their relatives.
