BAL并局部注入抗生素治疗支气管扩张合并感染的疗效观察

目的探讨支气管肺泡灌洗（BAL）并局部注入敏感抗生素治疗支气管扩张合并感染的临床疗效。方法62例支气管扩张合并感染患者随机分为治疗组和对照组，对照组给予常规治疗，治疗组在常规治疗的基础上给予BAL并局部注入敏感抗生素治疗。治疗前后测定动脉血氧分压（PaO2）、二氧化碳分压（PaCO2）、血氧饱和度（SaO2）血气分析指标；疗程结束后评价临床疗效；痊愈出院时计算住院时间和住院费用。结果治疗组临床疗效总有效率为96．9％，显著高于对照组76．7％（P〈0．05）；两组患者血气分析指标显著上升，与治疗前比较均有显著性差异（P〈0．05），且治疗组与对照组比较血气指标上升更显著（P〈0．05）；治疗组住院时间和总住院费用显著低于对照组（P〈0．05）。结论BAL并局部注入敏感抗生素治疗支气管扩张合并感染临床效果明显，值得临床上推广应用。

Clinical observation of broncho-alveolar lavage combined with antibiotic intervention for bronchiectasis

Aim To evaluate the therapeutic effects of broncho-alveolar lavage （BAL） combined with antibiotic intervention for bronchieetasis. Methods 68 cases of bronchieetasis patients were randomly divided into treatment group and control group, control group was given conventional therapy, on the basis of conventional therapy the treatment group was given BAL combined with antibiotic intervention. The blood-gas targets of PaO2, PaCO2 and SaOz of the two group were determined and the clinical efficacy was evaluated. The hospitalization stay and hospitalization costs at discharge were calculated. Results The total effective rate of treatment group was 96.9% ,it was significantly higher than that of the control group （P〈0.05）;Afler treatment the blood-gas targets in patients both groups improved significantly,compared with the pre-treatment were significantly different （P〈0.05）, and the treatment group compared with the control group there was significant difference （P〈0.05）;The hospitalization stay and hospitalization costs of the treatment group at discharge was significantly lower than that of the control group （P〈0.05）. Conclusion BAL combined with antibiotic intervention for bronchiectasis has significant clinical effects and is worthy of clinical application.
