有创机械通气在急性左心衰肺水肿中的临床应用

目的研究有创机械通气在急性左心衰肺水肿中的临床应用。方法分析我院18例因急性左心衰合并重症肺水肿住院的患者,在有创机械通气前后的临床症状及pH、PaO2、PaCO2、SaO2、心率、呼吸频率等各项监测指标的变化。结果18例患者中除1例在机械通气第3d出现心脏骤停抢救无效死亡外,其余17例心衰均得到很好的纠正,机械通气救治急性左心衰肺水肿的总有效率达94.4%。气管插管、呼吸机治疗后与治疗前比各项监测指标变化均P<0.05。结论对于急性左心衰造成的重症肺水肿患者,尽早应用有创机械通气可以提高抢救成功率。

Clinical application of traumatic mechanical ventilation in lung edema caused by left heart failure

Objective To study the clinical effects of traumatic mechanical ventilation in lung edema caused by left heart failure.Methods 18 cases of patients of severe lung edema caused by acute left heart failure in our hospital were analysed,including changes of criteria variation of clinical symptoms and PH,PaO2,PaCO2,SaO2,heart rate,respiration rate etc surrounding traumatic ventilation.Results Except one case died in the third day after mechanical ventilation by heart sudden stop and failure of resucitation.The other 17 cases heart failure all recovered successfully. The effective rate of mechanical ventilation in the therapy of lung edema caused by left heart failure reached 94.4%.Different criteria of tracheal intubation and respiratory machine before and after all shown P<0.05.Conclusion For patients of severe lung edema caused by left heart failure should use traumatic mechanical ventilation to increase success rate in treatment as soon as possible.
