颅脑外伤急性期低钠血症的临床分析

摘    要:目的探讨中枢性低钠血症的病理机制及治疗。方法对68例急性颅脑外伤(ACI)患者血中心钠肽(ANP)、脑钠肽(BNP)、内源性类洋地黄物质(EDLS)、抗利尿激素(ADH)及血尿钠浓度、血尿渗透压作同步测定。结果68例ACI患者中有27例出现低钠血症,且多集中出现在重伤或特重伤病员中。结论中枢性低钠血症与下丘脑———垂体轴功能受损致EDLS、ADH分泌增加相关,血ANP、BNP浓度降低未直接影响到血钠浓度。静脉给予外源性促甲状腺素释放激素(TRH)可以对抗ACI患者ADH分泌增加所引起的稀释性低钠血症。

Abstract:Objective This study aims at exploration of mechanism and therapy of hyonatremia with craniocerebral injury. Methods The changes of atrial natriuretic peptide(ANP), brain natriuretic peptide(BNP), endogenous digitalis-like substance(EDLS), antideuretic hormone(ADH), and serum Na , urine Na , plasma-osmolailty, urine-osmolality were observed in 68 patients with acute craniocerebral injury(ACI). Results6BZThere were 27 hponatremia in 68 patients with ACI and they were found mostly in severe of fetal patients. Conclusion Hyponatremia might be related to abnormal secretion of EDLS and ADH as the result of the hypothalamic-hypophysial system injury, not related to decrease of ANP and BNP. TRH might inhibit water-saline preference by antagonising the increase of ADH.
