老年急性心力衰竭患者血清NT-proBNP水平及APACHEⅡ评分与预后的相关性分析

目的探讨老年心力衰竭(以下简称心衰)患者血清NT-proBNP水平、APACHEⅡ评分与预后的相关性。方法测定97例ICU心衰患者血清NT-proBNP水平和进行急性生理学及慢性健康状况评分(APACHEⅡ评分)。以APACHEⅡ评分10分和20分为界分为3个组别。比较各组与NT-proBNP水平的关系。另外将生存组和死亡组分别作血清NT-proBNP水平及APACHEⅡ评分比较。结果 APACHEⅡ评分21～30组血清NT-proBNP水平明显高于11～20分组(P0.05),APACHEⅡ评分30分组动脉血清NT-proBNP水平明显高于20分组(P0.05)。NT-proBNP水平与APACHEⅡ评分相关,死亡组NT-proBNP水平明显高于存活组。结论血NT-proBNP检测方便、快捷与APACHEⅡ评分结合,可更客观反映老年心衰患者的预后。

Correlative analysis of plasma N-terminal pro-brain natriuretic peptide level, APACHE II score and prognosis of elderly patients with acute heart failure

Objective To investigate the correlation of plasma N-terminal pro-brain natriuretic peptide（NT-proBNP） level,APACHE Ⅱ score and prognosis of elderly patients with acute heart failure.Methods Ninety-seven old patients with acute heart failure who were hospitalized in ICU were determined NT-proBNP level and APACHE Ⅱ score.According to the value of APACHE Ⅱ score,all of patients were divided into three groups： 10-20（group one）,20-30（group two） and 30 above（group three）.And the correlation between APACHE Ⅱ score of every group and NT-proBNP level were observed.Additionally,the correlation between APACHE Ⅱ score and NT-proBNP level in survival groups or dead groups.Results NT-proBNP level of group two was significantly higher than that of group one（P 0.05）,and group three was significantly higher than that of group two（P 0.05）.NT-proBNP levels were related with APACHEⅡ score and NT-proBNP level of death groups were significantly higher than that of survival group.Conclusion Serum level of NT-proBNP could be determined conveniently and rapidly.It can objectively reflect the progrosis of elderly patients by determining both NT-proBNP level and APACHE Ⅱ score.
