cTnI、AST及CK在鉴别诊断大鼠骨骼肌与心肌损伤应用价值的比较

摘    要:目的探讨心肌肌钙蛋白Ⅰ(cardiac troponin Ⅰ,cTnI)在鉴别诊断大鼠骨骼肌与心肌损伤应用价值。方法 32只SD大鼠随机分为4组:心肌损伤组,单次背部皮下注射异丙肾上腺素;骨骼肌损伤组,单次单侧胫骨肌注射布比卡因;联合损伤组,单次单侧胫骨肌注射布比卡因及背部皮下异丙肾上腺素;正常对照组,不给予任何药物。予给药后24h,检测血清AST,CK及cTnI,常规HE染色观察骨骼肌和心肌受损情况。结果心肌损伤组和联合损伤组血清cTnI均显著高于正常对照组和骨骼肌损伤组(P<0.01),而联合损伤组和心肌损伤组AST与骨骼肌损伤组无明显差异(P>0.05),心肌损伤组CK与骨骼肌损伤组无明显差异(P>0.05)。结论cTnI在鉴别大鼠心肌及骨骼肌损伤上较AST、CK有较高的敏感性,并且在一定程度上能够降低误诊及漏诊率。

Compavsion with application value of cTnI,AST and CK in the differential diagnosis of skeletal muscle and myocardial injury in rats

Abstract:Objective To investigate the application value of cardiac troponin I(cTnI) in the differential diagnosis of skeletal muscle with myocardial injury in rats.Methods 32 SD rats were randomly divided into four groups,myocardial injury group,a single subcutaneous injection of isoproterenol;skeletal muscle injury group,a single unilateral tibial muscle injection of bupivacaine;combined injury group,a single unilateral tibial muscle injection of bupivacaine and subcutaneous isoproterenol.The normal control group,was not given any drug.Detection of serum AST,CK and cTnI,HE staining observed in skeletal and cardiac muscle damage after administration of 24h.Results Serum cTnI of rats in the myocardial injury group and the combined injury group was higher than that in control group and skeletal muscle injury group(P<0.05);There was no difference in combined injury group and myocardial injury group with skeletal muscle injury group(P>0.05) at AST,meanwhile,serum CK of rats in myocardial injury group and skeletal muscle injury group had no significant difference(P>0.05).Conclusion CTnI has higher sensitivity than AST,CK in the diagnosis of skeletal muscle and myocardial injury in rats,which is contributed to make the right diagnosis and avoiding delayed diagnosis in a certain extent.
