肺叶切除术中采用不同方式切口的对比研究

目的比较肺叶切除术中传统切口、腋下小切口与电视胸腔镜辅助小切口的疗效。方法对传统后外侧切口、腋下小切口及电视胸腔镜辅助小切口肺叶切除术中患者的手术时间、术中出血量、术后3d的胸腔引流量、引流管留置时间、术后第1d疼痛强度评分、住院时间、并发症发生率进行比较。结果胸腔镜辅助小切口组在术中出血量、术后3d的胸腔引流量、引流管留置时间、术后第1d疼痛强度评分、住院时间、并发症发生率优于腋下小切口与传统切口组（P〈0.05）。结论肺叶切除术中采用胸腔镜辅助小切口比传统手术方式更优。

Comparative study of lobectomy incisions used in different ways

Objective To compare efficacy of lobectomy traditional incision, axillary incision and with a small incision VATS. Methods All index of the traditional p0sterolateral incision, axillary incision and VATS lobectomy small incision in the patient＇s operative time, blood loss, postoperative 3d chest drainage, drainage tube indwelling time, postoperative pain intensity ld score, duration of hospitalization, the incidence of complications were compared respectively. Results Blood loss, postoperative 3d chest drainage, drainage tube indwelling time, postoperative pain intensity scores ld, hospital stay, incidence of complications of Assist- ed thoracic small incision group was significantly differences compareing with than axillary incision with the traditional incision group （P〈0.05）. Conclusion Lobectomy used in assisted thoracic small incision will be better than the traditional surgical approach.
