签约家庭医生式服务对社区高血压患者的影响临床观察

目的：观察社区高血压患者签约家庭医生式服务前后高血压知晓率、治疗率、控制率的变化。方法：选取社区高血压患者102例，根据“自愿、知情、选择”的原则，签约家庭医生式服务，对其进行干预及治疗。采取问卷调查形式进行签约前及签约1年后高血压知晓率、治疗率、控制率测评。结果：签约前高血压知晓率31．4％、治疗率24．5％、控制率15．7％；签约1年后高血压知晓率100．0％、治疗率98．0％、控制率84，3％；签约1年后较签约前高血压知晓率、治疗率、控制率分别提高68．6％、73．5％、68．6％，差异具有统计学意义。结论：签约家庭医生式服务显著提高高血压知晓率、治疗率、控制率，改善患者血压控制情况及生活质量，值得在社区广泛开展。

The Signing Family Doctor-service on Hypertensive Patients in Community

Objective： To observe the changes of awareness, treatment and control rates community hypertensive patients contract before and after the family doctor services. Method：Selected 102 eases of patients with hypertension, according to the principle of voluntary, informed j choose signed family doctor-service, intervention and treatment. Took the form of ques- tionnaires before signing contract 1 year after hypertension awareness, treatment rate, the evaluation of the control rate. Result： 31.4% of the results before signing hypertension awareness, treatment rate of 24.5% , 15.7% of the control rate ; 100.0% of the contract 1 year after hypertension awareness, treatment rate of 98.0% and 84.3% of the control rate; contract one year more before signing hypertension awareness, treatment, and control rates were increased by 68.6%, 73.5%, 68.6%, There＇ s a statistically significant difference. Conclusion： Signing family doctor services significantly improve hypertension awareness, treatment and control rates, improve the conditions and quality of life of patients with blood pressure control, it is worth widely carried out in the community.
