胰腺肿瘤九种神经内分泌标记物的观察

作者应用９种神经内分泌（Ｎｅｕｒｏｅｎｄｏｃｒｉｎｅ，ＮＥ）标记物，采用ＰＡＰ免疫细胞化学法检测２８例胰腺癌及７例胰岛细胞瘤，结果发现：腺癌中５例（１７．９％）含有铬粒素阳性ＮＥ细胞，呈激素阳性者２例，１例为血清素、胰高血糖素、胰多肽阳性，另１例为单一的胰多肽阳性；胰岛细胞瘤中呈铬粒素阳性ＮＥ细胞５例（７１．４％），含有激素产物３例，其中１例含有单一胰多肽，另２例分别含血清素、胰岛素、胃泌素和胰高血糖素、胰多肽。结果显示，胰癌中ＮＥ细胞发现率在高分化癌明显高于低分化癌（Ｐ＜０．０５），但中分化癌与高、低分化癌之间无明显差异（Ｐ＞０．０５）。作者对ＮＥ细胞的发现率与临床病理的关系，及铬粒素在诊断神经内分泌肿瘤中的实用价值进行了讨论。

STUDY OF 9 NEUROENDOCRINE MARKERSIN PANCREATIC TUMORS

Twenty eight cases of pancreatic carcinomas and 7 cases of islet cell tumors were studied by means of 9 different neuroendocrine(NE) markers with the PAP method. Five cases(17.9%)with chromogranin positive were detected in pancreatic carcinomas,among which 2 were foune to have positive hormones,one for serotonin,glucagon and pancreatic polypeptide and another for pancreatic polypeptide. Positive NE cells were found in 5 cases (71.4%)of islet cell tumors,among which 3 cases were hormone positive(1 for pancreatic polypeptide,2 for serotonin,insulin,gastrin,glucagon and pancreatic polypeptide).Results showed a higher incidence of NE cells in well-differentiated carcinomas than in poorly differentiated ones ( P <0.05).No statistic differences were observed between moderately and well differentiated cancers and moderately and poorly differentiated ones(P>0.05).The incidence and clinicopathological significance of NE cells found in the tumors and usefulness of chromogranin in the diagnosis of neuroendocrine tumors are discussed.
