摘    要:目的研究特应性皮炎(Atopic Dermatitis,AD)患儿外周血中调节性B细胞(Breg)的变化,分析其与Th22、Th17的关系,并探讨在AD患儿免疫学机制中的作用。方法选择28例AD患儿,分离外周血单核细胞(PBMC),采用流式细胞术(FCM)检测调节性B细胞与Th22、Th17的变化,用ELISA测定培养上清中IL-10、IL-22及IL-17水平,与同期选择的健康对照组比较。结果 AD患儿PBMC中CD19+IL-10+调节性B细胞数量低于健康对照组(1.15±0.31)%vs(2.36.±0.82)%,P0.05],Th22、Th17细胞的数量均高于健康对照组,培养液上清中细胞因子IL-10的水平也低于健康对照组,IL-22与IL-17水平高于对照组,差别有统计学意义(P0.05)。结论 AD患儿外周血调节性B细胞与以及相关的细胞因子IL-10的的下调,以及Th22、Th17细胞上调,可能共同参与了AD的免疫学机制,它们免疫紊乱的调节可为AD的临床治疗提供了新思路。

Abstract:Objective To detect the levels of regulatory B ceils and its relationship with Th17 and Th22 in PBMC and explore their significances in patients with atopic dermatitis(AD)children. Methods Peripheral blood mononuclear ceils (PBMC) were obtained from 28 AD children and 30 normal human controls. Flow cytometry was performed to detect the change of regulatory B cells with Th17 and Th22. ELISA was applied to determine the serum levels of IL-10 IL-22 and IL-17. Results A statistical decrease was observed in the numbers of peripheral CD19+IL-10t Breg cells in AD patients than normal control(1.15±0.31)% vs (2.36±0.82)%, P〈0.05], there was a significant increase in the number of Th17 and Th22 in patients compared with controls(P〈0.05), the expression levels of IL-10 in AD were significant lower than those of controls ( P〈0.05), a significant increase was observed in the levels of IL-22 and IL-17 compared with controls ( P〈0.05).Conclusion The lower expression of Bregs and the higher expression of Th17 and Th22 in AD patients suggest that Th22/Breg and Th17/Breg may play an important role in the pathogenesis of AD, which can foster a new idea for therapy of AD children.
