健康教育临床护理路径对住院精神分裂症患者及家属同步实施对照研究

摘    要:目的 探讨健康教育临床护理路径对住院精神分裂症患者及家属的同步实施效果。方法 选取2011年1月~2014年2月温州康宁医院住院精神分裂症患者200例,采用数字随机法分为两组,100例患者采用传统方法行健康教育为对照组,100例患者采用临床护理路径行健康教育为观察组,比较两组患者的遵医行为、住院情况、相关知识的掌握情况、满意度情况。结果 观察组患者遵医行为评分[(17.30±1.68)分]、相关知识掌握评分[(26.71±2.78)分]、满意度评分[(98.01±2.62)分]、护士用住院患者观察量表评分[(174.85±25.46)分]均明显高于对照组[(11.78±1.34)、(18.22±2.53)、(89.31±3.12)、(148.36±25.81)分],差异均有统计学意义(t=7.685,P=0.014;t=7.659,P=0.014;t=4.315,P=0.047;t=4.756,P=0.043)。观察组患者住院时间[(53.71±10.23)d]、住院总费用[(5876.34±463.67)元]均明显少于对照组[(69.83±12.41)d、(7525.13±612.51)元],差异均有统计学意义(t=5.986,P=0.031;t=5.806,P=0.032)。观察组患者阳性和阴性症状量表评分[(35.13±6.42)分]明显低于对照组[(46.58±5.27)分],差异有统计学意义(t=6.135,P=0.029)。结论 健康教育临床护理路径可显著提高住院精神分裂症患者及家属相关知识的认识水平和了解程度,可显著提高患者遵医行为,可明显缩短患者的住院时间,可显著降低患者的住院费用,具有明显的社会经济效益,值得临床推广使用。

Abstract:Objective To investigate synchronization implementation effect of heahh education clinical nursing pathway in patients with hospitalization schizophrenic and their families. Methods 200 patients with hospitalization schizophrenic in Wenzhou Kangning Hospital from January 2011 to February 2014 were randomly divided into two groups. 100 pa- tients treated with traditional methods of health education were as control group. 100 patients treated with clinical nursing pathway for health education were as observation group. Compliance behavior, hospitalization situation, related knowledge grasp situation, satisfaction were compared between the two groups. Results Compliance behavior score [(17.30±1.68) points], relevant knowledge master score [(26.71±2.78) points], satisfaction score [(98.01±2.62) points], NOSIE score [(174.85±25.46) points] in observation group were significantly higher than those of control group [(11.78± 1.34), (18.22±2.53), (89.31±3.12), (148.36±25.81) points], the differences were statistically significant (t = 7.685, P = 0.014; t = 7.659, P = 0.014; t = 4.315, P = 0.047; t = 4.756, P = 0.043). Hospitalization time [(53.71±10.23) d], total cost of hospitalization [(5876.34±463.67) yuan] in observation group were significantly less than those of control group [(69.83±12.41) d; (7525.13±612.51) yuan], the differences were statistically significant (t= 5.986, P = 0.031; t = 5.806, P = 0.032). PANSS score [(35.13±6.42) points] in observation group was significantly lower than that of control group [(46.58±5.27) points], the difference was statistically significant (t = 6.135, P = 0.029). Conclusion Clinical nursing pathway for health education can significantly improve relevant knowledge awareness level and understanding degree of patients with hospitalization schizophrenic and their families, which can significantly improve compliance behavior of patients and shorten hospitalization time.
