改良切口两线减张鼻中隔成形术治疗鼻中隔偏曲的效果

目的探讨改良切口两线减张鼻中隔成形术在治疗鼻中隔偏曲的效果。方法回顾分析2011年1月~2013年6月广西壮族自治区河池市人民医院（以下简称＂我院＂）耳鼻喉科收治的行改良切口两线减张鼻中隔成形术的中隔偏曲患者63例（改良组）及2009年1月~2011年12月我院行鼻中隔黏膜下切除术的鼻中隔偏曲患者63例（对照组）的临床资料。比较两组患者的手术时间、术中出血量、术后疼痛、鼻中隔黏膜损伤情况及治疗效果等。结果改良组治愈率（74.60%）、有效率（95.24%）均高于对照组（47.62%、84.13%）,差异均有统计学意义（P〈0.05或P〈0.01）;改良组患者手术时间（58.0±7.8）min]、术中出血量（50.0±6.7）mL]、术后疼痛（7.94%）、鼻中隔黏膜损伤（11.11%）情况明显优于对照组（70.0±7.7）min、（61.0±8.2）mL、20.63%、26.98%],差异均有统计学意义（P〈0.05或P〈0.01）。结论改良切口两线减张鼻中隔成形术,切口张力低、术野清晰、手术时间短、出血量少、并发症少、治疗效果佳,是一种可以选择的手术方案。

Effect of the two line tension relaxing septoplasty in the treatment of deviation of nasal septum

Objective To explore the therapeutic effect of modified incision septoplasty with two lines decreasing tension for deviation of nasal septum. Methods Clinical data of 63 cases of patients with nasal septum from January 2011 to June 2013 were treated by two lines decreasing tension in the Department of Otolaryngological, the People＇s Hospital of Hechi City in Guangxi Zhuang Autonomous Region （＂our hospital＂ for short） （improved group） and 63 patients with deviation of nasal septum from January 2009 to December 2011 were treated by submuscous resection of nasal septum in our hospital （control group） were retrospectively analyzed. The operation time, intraoperative blood loss, postoperative pain, nasal septum mucosa injury and therapeutic effect of two groups were compared. Results The cure rate and effective rate of the improved group （74.60%, 95.24%） were significantly better than those of the control group （47.62%, 84.13%）, the differences were statistically significant （P 〈 0.05 or P 〈 0.01）. The operation time, intraoperative blood loss, postoperative pain, nasal septum mucosa injury of the improved group （58.0±7.8） min, （50.0±6.7） mL, 7.94%, 11.11%] were better than those of the control group （70.0±7.7） min, （61.0±8.2） mL, 20.63%, 26.98%], the differences were statistically significant （P 〈 0.05 or P 〈 0.01）. Conclusion Modified incision septoplasty with two lines decreasing tension is a kind of operative plan can be choose, it has advantages in low incision tension, clear operative field, short operation time, less bleeding, low complication rate and satisfactory effect.
