支撑喉镜与电子喉镜治疗声带良性病变的效果及安全性比较

目的探讨支撑喉镜与电子喉镜治疗声带良性病变的效果及安全性。方法选取2011年6月～2013年6月绍兴市上虞人民医院耳鼻咽喉科住院并行手术治疗的声带良性病变患者96例。采用随机数字表法将入组的患者分为支撑喉镜组和电子喉镜组,每组各48例。支撑喉镜组予以支撑喉镜下声带良性病灶切除术,电子喉镜组予以电子喉镜下声带良性病灶切除术。观察并比较两组患者的手术时间、术中出血量、治疗效果及并发症发生情况。结果电子喉镜组患者手术时间和术中出血量均少于支撑喉镜组（t=8.71、2.48,P〈0.05或P〈0.01）;术后4周,两组患者治疗后的临床总有效率比较差异无统计学意义（χ^2=0.26,P〉0.05）;支撑喉镜组患者的术后并发症发生率明显少于电子喉镜组（χ^2=4.80,P〈0.05）。结论支撑喉镜与电子喉镜治疗声带良性病变效果相近,电子喉镜操作相对简单、手术时间短、术中出血量少、术后恢复快,而支撑喉镜手术切除彻底、术后并发症少。对于声带病灶较大或对术后发声要求较高的患者应选用支撑喉镜治疗。

Curative effect and security comparison of prop-up laryngoscope and electronic laryngoscope treatment for vocal cord benign lesions

Objective To discuss the curative effect and security of prop-up laryngoscope and electronic laryngoscope treatment for vocal cord benign lesions. Methods 96 cases of patients with vocal cord benign lesions, who were in Department of ENT, Shangyu People＇s Hospital of Shaoxing City and given operative treatment during the period from June 2011 to June 2013, were selected and divided into prop-up laryngoscope group and electronic laryngoscope group by random number table, with 48 cases of patients in each group. The patients in prop-up laryngoscope group were given vocal cord benign lesions resection by prop-up laryngoscope, while the patients in electronic laryngoscope group were given vocal cord benign lesions resection by electronic laryngoscope. The operation time, amount of bleeding during the operation, treatment effect and occurrence of complication of patients in the two groups were observed and compared. Results The operation time and amount of bleeding during the operation of patients in electronic laryngoscope group were much shorter or less than those in prop-up laryngoscope group（t = 8.71, 2.48, P〈0.05 or P〈0.01）.After comparing the total clinical efficiency of patients in two groups 4 weeks after operation, no obvious statistical differences were appeared（χ^2= 0.26, P〉0.05）. The postoperative complication occurrence rate of patients in prop-up laryngoscope group was much lower than that in electronic laryngoscope group（χ^2= 4.80, P〈0.05）. Conclusion The curative effects of prop-up laryngoscope and electronic laryngoscope for vocal cord benign lesions are similarly the same, and the electronic laryngoscope has relatively simpler operation, shorter operation time, less amount of bleeding during the operation, and quicker recovery after the operation, while the prop-up laryngoscope has more complete excision with less complication after the operation.
