脑转移瘤的X刀治疗与常规手术疗效比较

目的比较脑转移瘤的 Ｘ刀治疗与常规手术治疗的疗效及特点。方法在１１６例脑转移瘤患者中,对４７例采用 Ｘ刀治疗、 ６９例作常规手术治疗;其中４例联合运用两种方法。结果无围手术期死亡患者：随访全部病例两组生存期在６个月内者分别为１５％（４／４７）和１６％（１１／６９）,生存期达６个月以上者为８５％和８４％。结论Ｘ刀及常规手术治疗脑转移瘤各有其临床特点,颅内多发、新生及复发肿瘤仍适应于Ｘ刀治疗,两种方法结合使用可以明显增加脑转移瘤的治疗机会,延长晚期肿瘤患者的生存期,减少恶性肿瘤的颅脑源性死亡率。

Comparison of X-knife treatment and conventional operation for brain metastases

Objective To evaluate the therapeutic effect of X-knife and conventional operation in the treatment of brain metastases. Methods In 116 patients with brain metastases, 47 underwent X-knife treatment and 69 underwent conventional operation. 4 of them received both treatments. Results Follow-up of all cases showed that no death occurred during perioperation period. In patients undergoing X-knife treatment, 7 (15%) survived less than 6 months and 40 (85%) over 6 months. However, in the patients receiving conventional operation, 11 (16%) survived less than 6 months and 58 (84%) over 6 months. Conclusion Xknife treatment and conventional operation for brain metastases have their own clinical advantages. X-knife treatment is eligible for multiple tumor. newbom tumor and recurrent tumor. Combination of the two treatments could increase the chances to cure brain metastases, prolonging the survival period of patients with advanced malignant tumor and decrease the cranioencephlogenic mortality rate of malignant tumor.
