视网膜色素变性的吲哚青绿血管造影特征

摘    要:目的研究视网膜色素变性(retinitis pigmentosa,RP)的脉络膜血液循环特征。方法对37例RP患者行吲哚青绿血管造影(indocyanine green angiogra phy,ICGA),并与正常人眼对照。结果①RP组眼底荧光最早出现的平均时间(14.38±3.95) s,脉络膜静脉荧光开始充盈的平均时间(17.27 ±5.94) s,与对照组比较差异无显著性的意义(P>0.05)。 ②RP组脉络膜血管荧光减弱的平均时间(475.75±153.70) s,较对照组明显缩短(P<0.0001)。③RP组眼底后部强荧光区的平均面积(41.20±19.99) mm2,较对照组明显缩小(P<0.0001)。④ICGA中后期RP组纱幕状荧光缺如61只 眼,占84.7%;斑状强荧光21只眼占29.2%,4只眼强荧光点渗漏,占5.6%。结论①RP脉络膜血管灌注压无改变;②RP脉络膜血流量减少;③RP脉络膜毛细血管萎缩;④RP可伴有脉络 膜新生血管形成。(中华眼底病杂志,2001,17:26-29)

Abstract:Objective To study the characteristics of choroidal circulation in RP. Methods Using ICGA to observe 37 cases of RP and compare with healthy volunteers. Results ① The earliest fluores-cein filling time of the choroidal arteries in RP group was (14.38±3.95) seconds,the choroidal veinous in RP group was (17.27±5.94) seconds,and there was no obvious difference between RP and control group.②The fluorescein failing time of choroidal vein in RP group was (475.75±153.70)seconds.③The area of the bright fluorenscence in posterior fundus in RP group was (41.20±19.99) mm2,and compared with the control group,there was significant difference (P<0.0001).④In the mid to late phase during ICGA,in RP group the veil-like hypofluorescence was found in 61 eyes (84.7%),plaque hyperfluorescence in posterior fundus in 21 eyes (29.2%),and leakage of heperfluorescence in 4 eyes(5.6%). Conclusion ①The perfusion pressure of choroidal vessels in RP reveals no change.②The blood volume of choroidal vessels becomes decreased in RP.③The choroidal capillaries become atrophic in RP.④Choroidal neovascularization may occur in patients with RP.
