可吸收螺钉联合克氏针固定治疗部分踝关节骨折的疗效评价

目的 探讨可吸收螺钉联合克氏针固定治疗部分踝关节骨折的疗效. 方法 2003年7月至2007年5月.对42例踝关节骨折患者采用可吸收螺钉联合克氏针固定治疗部分踝关节骨折,男27例,女15例;年龄20～70岁,平均39.2岁.术中于可吸收螺钉附近加用1～2枚克氏针,术后4周左右,于门诊拔除克氏针,后逐渐行功能锻炼,分析其治疗效果. 结果 所有患者术后平均随访24个月(14～38个月).骨折全部骨性愈合,无一例出现骨折移位、伤口感染等现象.根据英国足踝外科协会(AOFAS)制定的踝关节评定标准:优35例,良6例,可1例,优良率达97.6%.结论可吸收螺钉联合克氏针固定治疗部分踝关节骨折,骨折愈合率高,踝关节功能恢复良好,并可免除二次手术取内固定,值得推广.

Treatment of ankle joint fractures with absorbable screws and Kirschner wire

Objective To discuss treatment of the ankle joint fractures with absorbable screws and Kirschner wire. Methods From July 2003 to May 2007, 42 patients with ankle joint fracture were treated with absorbable screws and Kirschner wire. Fractures were fixed with 1 to 2 Kirschner wires around the absorbable screws in operation, and plaster slabs were applied at functional position for 4 to 6 weeks after operation. Their Kirschner wires were removed in outpatient service about 4 weeks after operation and func-tional exercise was encouraged gradually. Results All the 42 cases were followed up for an average of 24 months. All the fractures healed without nonunion and infection According to the evaluation criteria of American Orthopaedic Foot and Ankle Society (AOFAS), the results were rated as excellent in 35 cases, good in 6, mod-erate in 1, and poor in none. The total good to excellent rate was 97.6%. Conclusion Absorbable screws and Kirschner wire can be applied for the ankle joint fracture, leading to satisfactory healing rate and good ankle functional recovery, as well as sparing the patient secondary operation.
