摘    要:目的研究二尖瓣脱垂患者成形术前、术后瓣叶及瓣环各参数的改变,探讨各参数与二尖瓣反流面积的关系。方法 2012年2月至2013年6月选取中日联谊医院心外科因单纯二尖瓣脱垂行二尖瓣成形术患者,20例患者入选。采用经食管三维超声探头存储图像,二尖瓣定量分析(MVQ)软件对二尖瓣成形术前、术后复跳即刻二尖瓣进行定量分析,分析参数包括:瓣环的前外侧至后内侧直径(DAIPm)、前结合长度(LCA3dLf)、后结合长度(LCP3dLf)、前中部小叶节段的长度(L2DDA2)、后中部小叶节段的长度(L2DDP2)、前小叶面积(A3DTant)、后小叶面积(A3DTpost)、最大脱垂高度(HProl)、小叶脱垂体积(VProl)。同时记录术前、术后复跳即刻的二尖瓣反流面积(MR)。结果术前MR、DAIPm、LCA3dLf、LCP3dLf、L2DDA2、L2DDP2、A3DTant、A3DTpost、HProl、VProl分别为(16.64±6.24)cm2、(43.58±7.72)mm、(42.66±10.00)mm、(45.45±10.89)mm、(24.00±5.91)mm、(17.09±3.82)mm、(1 227.28±392.72)mm2、(774.79±221.18)mm2、(7.07±4.13)mm、(1.29±1.17)ml,术后上述参数分别为(1.53±1.30)cm2、(31.15±4.12)mm、(29.93±8.53)mm、(29.77±7.95)mm、(21.41±2.94)mm、(9.18±3.54)mm、(731.66±162.40)mm2、(348.23±112.56)mm2、(3.30±1.85)mm、(0.18±0.16)ml,术后各参数均小于术前,差异均有统计学意义(t值分别为11.176、8.108、5.789、7.343、2.201、6.861、7.344、10.441、4.193、4.398,P均〈0.05)。术前LCP3dLf较LCA3dLf长,差异有统计学意义(t=-2.923,P〈0.05);术后LCA3dLf与LCP3dLf比较,差异无统计学意义(t=0.193,P〉0.05)。多元线性回归分析方程为:Y(MR)=-0.037+0.031X1(A3DTpost)-0.361X2(L2DDA2),L2DDA2、A3DTpost是MR的独立影响因素,且L2DDA2与MR成反比,A3DTpost与MR成正比。测试者内部及测试者之间MR、A3DTpost及L2DDA2的测量结果于Bland-Altman图上?

Abstract:Objective To research the changes of the parameters of mitral valve leaflets and mitral annulus before and after the mitral valvuloplasty, and to explore the correlations of mitral regurgitation area and the parameters. Methods Twenty patients with pure mitral valve prolapse who underwent mitral valvuloplasty in China-Japan Union Hospital from February 2012 to June 2013 were enrolled. Mitral valve images were acquired by real-time three-dimensional transesophageal echocardiography(RT-3D-TEE). Quantitative analyses of parameters such as anterolateral to posteromedial diameter of annulus(DAIPm), length of anterior combination(LCA3dLf), length of posterior combination(LCP3dLf), length of A2(L2DDA2), length of P2(L2DDP2), area of anterior leaflet(A3DTant), area of posterior leaflet(A3DTpost), height of prolapse(HProl), volume of prolapse(VProl) and mitral regurgitation area(MR) were carried out by mitral valve quantification(MVQ) software before and after the mitral valvuloplasty. Results Before the operation, MR, DAIPm, LCA3dLf, LCP3dLf, L2DDA2, L2DDP2, A3DTant, A3DTpost, HProl and VProl were(16.64±6.24) cm2,(43.58±7.72) mm,(42.66±10.00) mm,(45.45±10.89) mm,(24.00±5.91) mm,(17.09±3.82) mm,(1 227.28±392.72) mm2,(774.79±221.18) mm2,(7.07±4.13) mm and(1.29±1.17) ml. After the operation, the above parameters were(1.53±1.30) cm2,(31.15±4.12) mm,(29.93±8.53) mm,(29.77±7.95) mm,(21.41±2.94) mm,(9.18±3.54) mm,(731.66±162.40) mm2,(348.23±112.56) mm2,(3.30±1.85) mm and(0.18±0.16) ml. The latter parameters were significantly smaller than the former with significant differences(t=11.176, 8.108, 5.789, 7.343, 2.201, 6.861, 7.344, 10.441, 4.193 and 4.398, all P0.05). LCP3dLf was longer than LCA3dLf with significant differences before the operation(t=-2.923, P 0.05), but without significant difference after the operation(t=0.193, P 0.05).
