肩背部弹力纤维瘤的64排螺旋CT诊断

目的分析肩背部弹力纤维瘤的64排螺旋CT影像表现对临床诊断的价值。方法回顾性分析2008-2012年间我院诊治的11例肩背部弹力纤维瘤患者的64排螺旋CT及普通螺旋CT影像表现,总结特征,结合文献进行综合分析。结果螺旋CT平扫显示,11例肩背部弹力纤维瘤病例中,双侧发病7例（63.6%）,单侧发病4例（36.4%）,均表现为肩胛下角内侧前锯肌、背阔肌及菱形肌深层与背侧胸廓筋膜间规则实性肿物;肿物依大小差异分别呈盘形、扁丘形,以宽基底弧面状附着于胸廓筋膜,凸面朝向体外,相邻肌肉,周围脂肪间隙清晰;采用64排螺旋CT扫描的10例病例16处病灶均具有肿物密度不均匀,但密度差异有规律,主体呈等肌肉密度,内间杂条纹样的脂肪密度区域的特点;采用普通螺旋CT扫描的一例病例双侧病灶均未见明确清晰的脂性条纹征。结论肩背部弹力纤维瘤的64排螺旋CT扫描具有典型的影像学特征,典型患者结合临床表现可做出明确诊断,为患者的治疗提供准确可靠的依据。

Diagnosis of Elastofibroma Dorsi by 64-slice Spiral CT

Objective To investigate the diagnosis value of elastofibroma doris by 64-slice Spiral CT. Methods In Luhe hospital from 2008 to 2010, The CT image characteristic of 11 cases of elastofibroma dorsi were reviewed and comprehensively analyzed with literature. Results In 11 cases of elastofibroma dorsi with CT scan, the fibroid tumor of 7 cases （63.6%） occur in double side, 4 cases （36.4%） in single side, and all of the cases showed that the solid masses occurred between latissimus dorsi and thoracic fascia. The masses were shaped as half oval or plate type, and the bottom was on thoracic fascia, the nearby muscle was oppressed by lesions, and the tumors had definite margins surrounded by fat. 16 tumors of 10 cases by 64-slice spiral CT scanning had same histological feature. The mass density was uneven, but the density variation was regular. The mass had the equal density with muscle, and mixed with the fat-density region, bilateral tumors of 1 case by normal spiral CT scanning don＇t show fat-density region. Conclusion Typical image characteristics of elastofibroma dorsi by 64 line spiral CT scan, with the combination of patients＇ clinical manifestations, can provide the reliable evidence for the definite diagnosis.
