胸膜孤立性纤维瘤的多层螺旋CT诊断

目的探讨胸膜孤立性纤维瘤（SFTP）的多层螺旋CT（MSCT）表现,以进一步提高其认识和诊断。资料与方法回顾性分析11例经手术病理证实为SFTP的MSCT表现及临床资料,并复习文献。结果男性7例,女性4例,年龄34-63岁。11例病灶均为单发,紧贴胸膜;大小2.1-18.4cm,3例大于10cm。MSCT上肿瘤形态多呈类圆形,并可见浅分叶,边界清晰;密度多较均匀,以实性成分为主;4例伴囊变、坏死而密度不均,2例边缘见小钙化。增强8例呈较均匀轻-中度延迟性强化,3例呈较明显不均匀强化;4例病灶内部可见强化的迂曲小血管影。结论 SFTP的影像学表现具有一定的特征性,结合CT三维重建有助于提高其正确诊断,具有重要意义。

Multi-slice CT Diagnosis of Solitary Fibrous Tumor of the Pleura

Objective To analyze the multi-slice CT （MSCT） findings of solitary fibrous tumor of the pleura（SFTP） to further improve its recognition and diagnosis. Materials and Methods The MSCT findings and clinical data of 11 cases with pathologically proved SFTP were analyzed retrospectively and reviewed of literature. Results Of 11 patients, 7 cases were male, 4 were female, and with an age of 34-63 years old. 11 lesions were all single, close to the pleura, with size of 2.1-18.4cm in diameters, in which 3 were more than 10cm. On plain MSCT scans most of the tumor were roundish with mild lobulated and clear boundary; the density were homogeneous with the solid parts predominantly. 4 cases appeared inhomogenous density with cystic change and necrosis, 2 cases appeared small calcification on the edge of the lesion.8 cases appeared relatively homogeneous mild-moderately delayed enhancement while 3 cases obviously heterogeneous enhancement on MSCT enhancement scanning. And there were enhanced and tortuous small vessels were observed in 4 lessions. Conclusion The imaging findings of SFTP have certain characteristics, Combined with CT three-dimensional reconstruction, it can help to improve its correct diagnosis with great importance.
