IVU后CTU在输尿管病变的诊断价值

目的探讨静脉尿路造影（IVU）后CT尿路成像（CTU）检查的技术及其对输尿管病变的诊断价值。方法对泌尿系疾病患者行标准的IVU检查，39例疑输尿管病变的患者立即行CT检查，利用CT—SPR技术扫描定位像，扫描结束后在工作站上重建成CTU。分别让两位高年资影像医师对IVU、CT—SPR和CTU进行评价，分析其对病变的显示和病变的诊断。结合手术病理结果和临床诊治结果来分别判定其检出率和诊断正确率。结果本组研究39例输尿管病变患者中IVU能显示梗阻部位的显示为33例，占85％，另外6例因肾功能不良而无法判定，单纯CT-SPR与IVU结果相同，CT加CTU对梗阻部位的显示率为100％。整体诊断正确率CT加CTU为92％，而IVU为59％。在CTU与最终结果的比较中，P〈0．05，一致性较好。结论IVU后CTU对输尿管梗阻性疾病的检出率和诊断正确率都高于IVU。

CTU after IVU in evaluating ureteral diseases

Objective To evaluate the mode of the spiral CT uography, and the ability of diagnosing ureters diseases on CTU after IVU examination. Methods Thirty- nine patients underwent CT urography after IVU. Analyzed the performance of the ueters, compared the ability in diagnosing ureters diseases between CTU and IVU. Results 39 cases were confirmed as utrteral urolithiasis , neoplasm of ureter , tuberculosis , pelvis and ureter duplication, megalo ureter, et al. Both IVU and CTU can find almost all the locations of the obstructive site, CT - SPR has the same ability. But to evaluate the reasons of the obstruction , IVU has its limited. The accurate of CTU in diagnosing ureter diseases is up to 92%. Use SPSS for windows Ver. 11.5 to analyze the results. There is statistically significant between IVU and CTU . Conclusion The accurate rate of CTU after IVU is superior to that of simple IVU in diagnosing ureter diseases.
