蟾蚧温肾丸配合无创通气治疗慢性阻塞性肺疾病并发呼吸衰竭临床观察

目的：评价蟾蚧温肾丸配合无创通气治疗慢性阻塞性肺疾病并发呼吸衰竭的临床疗效及对血液流变学的影响。方法：120例慢性阻塞性肺疾病并发呼吸衰竭急性发作期患者，分成观察组和对照组各60例。对照组采用无创通气及对症支持治疗，观察组采用蟾蚧温肾丸佐助无创通气治疗，必要时对症支持治疗。观察慢性阻塞性肺疾病并发呼吸衰竭患者的症状体征变化，及血液流变学指标。结果：观察组总有效率95．00％，高于对照组总有效率83．33％，2组比较，差异有显著性意义（P〈0．05）。治疗前，2组患者血液流变学指标比较，差异无显著性意义（P〉0．05）。治疗后，观察组全血低切黏度、全血中切黏度、全血高切黏度及红细胞压积等指标的改善优于对照组，差异均有非常显著性意义（P〈0．01）。结论：蟾蚧温肾丸配合无创通气治疗慢性阻塞性肺疾病并发呼吸衰竭疗效可靠，具有良好的安全性，而且能有效的改善患者血液流变学指标。

Clinical Observation of Chanjie Wenshen Pill Assisted with Noninvasive Ventilation in Treatment of Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

Objective:To evaluate the therapeutic effect of Chanjie Wenshen pill assisted with noninvasive ventilation in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure and to observe the effect on blood rhe-ology.Methods:One hundred and twenty cases of chronic obstructive pulmonary disease complicated with respiratory failure patients were divided into the observation group and the control group.The control group received symptomatic treatment,and the observation group received Chanjie Wenshen pill assisted with noninvasive ventilation,and was given symptomatic treatment when necessary.The changes of symptoms and sighs as well as hemorheological indexes were observed.Results:The total effective rate of the observation group was 95.00%,higher than 83.33% in the control group,and the difference was statisti-cally significant(P < 0.05).The differences of himorheological indexes were insignificant between the two groups(P > 0.05).Af-ter treatment,low-shear viscosity,middle-shear viscosity and high-shear viscosity of the whole blood and hematocrit in the observation group were superior to those in the control group,the difference being insignificant(P < 0.01).Conclusion:Chanjie Wenshen pill assisted with noninvasive ventilation is safe and effective in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure,and is effective to improve the blood rheology.
