171例老年糖尿病患者中医证候特征与衰弱程度相关性的探讨

目的探讨老年糖尿病患者中医证候特征与衰弱程度的相关性。方法按照国家《中医临床诊疗术语·证候部分》的证候判定标准及病变脏腑诊断标准,同时应用《中西医结合老年衰弱评估量表》评估患者的衰弱指数,根据《临床衰弱量表》计算患者的衰弱分级情况,采用皮尔逊相关分析、斯皮尔曼分析、独立样本T检验及卡方检验比较不同证候的衰弱指数及衰弱分级情况。结果171例患者中常见的证候依次为阴虚、气虚、血瘀。患者兼夹证候越多,衰弱指数及衰弱等级便越高;患者病程越长,衰弱指数及衰弱等级便越高;患者并发症越多,衰弱指数越高;衰弱指数在血虚、阳虚、痰浊、血瘀、火热、毒证候下差异有统计学意义(P<0.05),而在气虚、阴虚、气滞、湿证候下差异无统计学意义(P>0.05);阴虚、气滞、火热、湿、毒各证在不同衰弱分级情况下差异无统计学意义(P>0.05),而气虚、血虚、阳虚、痰浊、血瘀证在不同衰弱分级情况下差异有统计学意义(P<0.05)。结论本研究提示老年糖尿病患者部分中医证候与衰弱程度密切相关。

Correlation between TCM syndrome and frailty in 171 elderly diabetic patients

Objective To explore the correlation between TCM syndromes and frailty in elderly diabetic patients.Methods Syndrome was differentiated according to the TCM Clinical Diagnosis and Treatment Terms·Syndrome syndrome judgment standard and pathological viscera diagnosis standard.The frailty index of the elderly patients was evaluated by the Frailty Scale of Integrated Traditional Chinese and Western Medicine,and the frailty grade of the patients was evaluated by the Clinical Frailty Scale.Pearson correlation analysis,Spearman correlation analysis,independent sample T-test and chi-square test were used to compare the relationship between different syndromes and frailty index and grade.Results The most common syndromes in the 171 patients were yin deficiency,qi deficiency,and blood stasis.The index and grade of frailty were higher when there were more syndromes or complications in patients,or the disease course was longer.There was statistical significance between the frailty index and the syndromes of blood deficiency,yang deficiency,turbid phlegm,blood stasis,fire-heat,and toxin(P<0.05).There was no statistical significance between the frailty index and the syndromes of qi deficiency,yin deficiency,qi stagnation,and dampness(P>0.05).In different grades of frailty,there was no statistical significance in the syndromes of yin deficiency,qi stagnation,fire-heat,dampness,and toxin(P>0.05).There was statistical significance in the syndromes of qi deficiency,blood deficiency,yang deficiency,turbid phlegm,and blood stasis(P<0.05).Conclusion Some TCM syndromes are closely related to the degree of frailty.
