祛痰利肺汤加减对慢性阻塞性肺炎急性加重期咳喘疗效及血清炎性因子的影响

目的观察祛痰利肺汤治疗慢性阻塞性肺炎急性加重期(痰浊壅肺)咳喘疗效及血清炎性因子的影响。方法纳入2017年1月—2019年1月期间慢阻肺急性加重期患者112例,其中治疗组56例,对照组56例,对照组常规止咳化痰解痉平喘(沙丁胺醇1 ml+异丙托溴胺2 ml)治疗;治疗组在对照组基础上给予祛痰利肺汤治疗,连续治疗7 d,观察治疗前后咳喘缓解率、中医证候量表评分、肺功能分级、咳嗽症状量表评分、血清炎性因子(TNF-α、IL-8)、不良反应。结果①两组慢阻肺急性加重期患者治疗后治疗组咳喘总缓解率为91.1%;对照组总缓解率为73.2%;②两组慢阻肺急性加重期患者治疗后中医证候量表评分、咳嗽症状评分、血清炎性因子(TNF-α、IL-8)、肺功能分级比较,差异有统计学意义(P<0.05);③治疗组患者未出现不良反应。结论祛痰利肺汤治疗慢阻肺急性加重期(痰浊壅肺)比单纯西医治疗效果显著,值得临床应用推广。

Effect of modified Qutan Lifei Decoction on cough,asthma and serum inflammatory factors in acute exacerbation of chronic obstructive pulmonary disease

Objective To observe the effect of Qutan Lifei Decoction on cough and asthma on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)(differentiated by turbid phlegm obstructing lung)and the effect on serum inflammatory factors.Methods A total of 112 AECOPD patients treated from January,2017 to January,2019 were included and divided into a treatment group(56)and a control group(56).The control group was given routine treatment of cough and asthma relieving(salbutamol 1ml+ipratropium bromide 2 ml).The treatment group was additionally given Qutan Lifei Decoction on the basis of control group.The treatment lasted for 7 days.The response rate of cough and asthma,TCM syndrome score,pulmonary function grade,cough symptom scale score,serum inflammatory factors(TNF-α,IL-8),and adverse reactions were compared before and after treatment.Results The overall response rate of cough and asthma was 91.1%in the treatment group and 73.2%in the control group.There was significant difference in the TCM syndrome score,cough score,serum levels of inflammatory factors(TNF-α,IL-8),and the grade of pulmonary function,with statistical significance(P<0.05).No adverse events were found in the treatment group.Conclusion For AECOPD(differentiated by turbid phlegm obstructing lung),the application of Qutan Lifei Decoction with western medicine has a better therapeutic effect than conventional western medicine.
