帕金森病抑郁状态的相关因素研究

目的研究帕金森病（Parkinson disease，PD）患者抑郁症状的发生率及其相关影响因素。方法2007年3月至2008年3月河南省人民医院神经内科门诊诊断为PD的患者121例，采用抑郁自评量表（self—rating depression scale，SDS）、汉密尔顿抑郁量表（Hamilton depression scale，HAMD）对患者进行评定及诊断，统计分析抑郁的发生情况和严重程度及相关影响因素。结果PD患者抑郁的发生率为43．8％（53／121），轻度抑郁占90．6％（48／53）；经济状况差、病程长、Hoehn＆Yahr分级（简称H—Y分级）高者易出现抑郁；logistic回归分析表明病程和H—Y分级是抑郁的危险因素（OR=11．31，P=0．001，95％CI=3．46～28．69；OR=7．52，P=0．001．95％CI=2．68～18．71）。结论PD患者有较高的抑郁发生率，与PD病程、病情分期密切相关。

Study on correlated factors in patients with Parkinson diseases

Objective To assess the incidence of depression and correlated factors in Parkinson disease. Methods One hundred and twenty-one patients were selected from the department in March 2007 to March 2008. The patients were evaluated with Self-rating Depression Scale（SDS） and Hamilton Depression Scale（HAMD）, the disease severity and correlated factors of depression were analyzed. Results The incidence of depression in Parkinson disease patients was 43.8%（53/121） ,of which 90.6%（48/53） was mild depression. Univariate analysis revealed that a high frequency of depression occurred in the patients with bad economic status, long Parkinson disease duration and high Hoehn Yahr stage. The Parkinson disease duration and Hoehn ＆ Yahr stage association achieved significance when multivariable logistic regression analysis was used for the risks of Parkinson disease depression （ OR = 11.31, P = 0.001,95% CI =3.46-28.69; OR =7.52, P =0.001,95% CI =2.68-18.71）. Conclusion Parkinson disease patients have a high susceptibility to depression,and duration of PD and Hoehn ＆ Yahr stage appear to be the strongest predictor of depression.
