B超下胸膜活检术在胸腔积液病因诊断中的价值

目的探讨38例为B型超声引导下胸膜活检在疑恶性胸腔积液病因诊断中的价值。方法选常规胸膜活检A组38例,B超引导下胸膜活检患者38例为B组,活检后的标本送病理检查。结果A组患者病理确诊率68.42%(26/38),未检出者有12例,阴性率为31.58%(12/38)。A组阴性者12例再改行B型超声活检,其中又有10例患者病理确诊;B组患者活检病理确诊为肿瘤33例,2例确诊为结核,总检出率为92.11%(35/38),未检出者有3例,阴性率7.89%(3/38)。结论胸腔积液病因诊断用B型超声引导下胸腔活检术优于常规胸膜活检术,能提高确诊率和检出率,其方法简单、安全、实用性强,值得推广使用。

Diagnostic value in suspected malignant pleural effusion under ultrasound B type guided to pleural biopsy

Objective To investigate diagnostic value in suspected malignant pleural effusion (SMPE) of ultrasound of B type guided to pleural biopsy (UGPB). Methods A total of 38 cases of SMPE using routine pleural biopsy (RPB) served as Group A,another 38 cases using UGPB served as Group B. The samples of both groups after operation were sent to pathology department for living pathological diagnosis. Results For Group A,the accuracy rate was 68.42% (n=26),the negative rate was 31.58% (n=12). Among the 12 cases,10 were tested positive using UGPB (83.33%). For Group B,the accuracy rate was 92.11% (n=35). Among the 35,33 were diagnosed as malignant pleural disease and 2 as pulmonary tuberculosis. The negative rate was 7.89% (n=3). Conclusion Use of UGPB in SMPE patients is better than RPB. UGPB increases accuracy rate. It is easy to use,safe,practical and worthy of being used widely in clinic.
