青光眼合并高度近視視野的改燮特徵

摘    要:目的研究合并高度近视的青光眼和非高度近视的青光眼的视野改变有无差别。方法 利用Octopus 101全自动视野计对36例(51眼)合并高度近视的原发性开角型青光眼(primary open angle glaucoma,POAG)和16例(23眼)非高度近视的POAG进行静态中心阈值视野检查,并分析其视野缺损形式,视野缺损与生理盲点及固视点的关系和视野指数的改变。结果合并高度近视的POAG视野缺损形式与非高度近视的POAG视野缺损形式相似,差异无显著性(早期POAG,X~2=0.000138,P>0.05,中晚期POAG,X~2=1.1494,P>0.05)。合并高度近视的POAG视野缺损较多的与生理盲点相速或/和与固视点相近(或相连),但与非高度近视的POAG相比,差异无显著性(早期POAG,X~2=1.3892,P>0.05,中晚期POAG,X~2=2.6852,P>0.05)。随着近视度数的增加,MS值逐渐下降,MD和LV值逐渐升高。结论 合并高度近视的POAG视野缺损较多的与生理盲点和/或固视点相连或相近,近视度数越高,其视野损害越明显。

Abstract:Objective To find out whether the visual field changes are different between primary open angle glaucoma (POAG) with high myopia and POAG without it. Methods The static central visual fields were tested by Octopus 101 automated perimeter in 36 cases (51 eyes) of POAG with high myopia and 16 cases (23 eyes) of POAG without it. The visual field defect pattern, the correlation a-mong the visual field defect, physiologic blind spot and central fixation spot, as well as the change in visual field defect indices were analyzed and compared. Results The visual field defect patterns of POAG with high myopia were similar to that of POAG without high myopia, (early POAG, x2=0.00.138,P>0.05; middle and late POAG, x2= 1.1494,P>0.05). The visual field defectarea in most of PAOG with high myopia group was linked with physiologic spot and/or linked with (closeto) central fixation spot. Concerning this, there was no significance difference hetuee the 2 groups (early POAG, x2=1.3892,P>0.05; middle and late POAG, x2=2.6852,P>0.05). With the increase in the degree of high myopia mean sensitivity (MS) decreased, and the mean defect (MD) and loss variance (LV) increased gradually. Conclusion The visual field defect in most of POAG with high myopia was linked with and/or near to with physiologic spot and central fixation spot. The higher the degree of high myopia is, the more severe the visual field defect is.
