外伤性十二指肠损伤的诊治

目的 探讨外伤性十二指肠损伤的处理及影响预后的因素。方法 对1988年1月至2005年月9月收治的21例外伤性十二指肠损伤病例的临床资料进行回顾性分析。结果术前确诊1例（4．76％），漏诊20例（95．2％），死亡4例（19．04％）；发生并发症6例（28．5％），十二指肠瘘和腹腔内脓肿是主要并发症。结论 十二指肠损伤术前确诊率低，早期剖腹手术，抗感染及营养疗法是降低并发症和死亡率的关键。

The diagnosis and treatment of traumatic duodenal injury

Objective To investigate the prognostic factors and managements of the duodenal trauma.Meth- ods Clinical data of duodena trauma from January 1988 to September 2005 were collected and retrospectively anal- ysed.Results One case(4.76%)got preoperative diagnosis,and 20(95.2%)missed and 4(19.04%)died,compli- cations occurred in 6(28.5%),duodenal fistula and abdomina abscess were the major complications.Conclusion The rate of confirmed diagnosis is quite low,and early laparotomy,anti-infection and surgical nutrition are the key factor to reducing death rates and its complications.
