摘    要:目的 评价1.5T MR肝脏快速容积采集序列(LAVA)增强扫描对判断前列腺癌的内部浸润分级、强化类型、周围组织浸润的价值及其检出部位与经直肠超声引导下穿刺活检的一致性.方法 回顾性分析38例经直肠超声引导下穿刺活检证实的前列腺癌患者的MRI资料,并分析前列腺癌的内部浸润分级与MR LAVA强化类型的关系及MR LAVA增强扫描对外周带前列腺癌部位显示的结果与经直肠超声引导下双侧活检结果的一致性.结果 前列腺癌的内部浸润分级与MR LAVA强化类型有关;随着内部浸润分级增加,肿瘤浸润周围结构的几率增加;MR LAVA增强扫描对外周带前列腺癌部位显示的结果与经直肠超声引导下穿刺双侧活检结果的一致性较好(Kappa=0.803).结论 LAVA增强扫描可以提供更多的直接征象和强化特征,有助于准确判断前列腺癌周围浸润的部位.

Abstract:Objective To investigate the clinical value of 1.5T MR liver acquisition with volume acceleration (LAVA) dynamic contrast-enhanced technique in prostate cancer on grade of infiltration, types of enhancement and infiltrated region, and the consistency of LAVA to ultrasound-guided biopsy. Methods The relationship between grade of infiltration and types of MR enhancement of 38 patients with prostate cancer were analyzed, so as the consistency between MR LAVA dynamic contrast-enhanced technique to transrectal ultrasound-guided biopsy on showing prostate cancer in peripheral zone. Results Positive correlation was found between grade of infiltration and types of MR enhancement. The probability of surrounding tissue infiltration of prostate cancer increased with infiltrated grade. There was good consistency between MR LAVA dynamic contrast-enhanced technique and transrectal ultrasound-guided biopsy on showing prostate cancer in peripheral zone (Kappa=0.803). Conclusion LAVA dynamic contrast-enhanced technique can provide direct signs and enhanced features, so as to accurately identify the infiltrated region of prostatic carcinoma.
