握力:一种评估腹膜透析患者营养状况的方法

目的 探讨腹膜透析患者的握力与营养状态的关系。方法 选择北京大学第一医院腹透中心患者235例，分析握力与其他营养指标的关系及其影响因素。结果 握力与体重、血清白蛋白、瘦体重、标准化的氮出现率相当蛋白等营养指标显著相关（P＜0．05）。对握力多因素逐步回归分析结果显示：年龄、性别对握力有显著影响（P＜0．05），血清CO2结合力、血清C-反应蛋白、标化细胞外液与握力无相关性（P＞0，05）。结论握力是一种简单易行且经济的测量方法，它能够较好地反映机体肌肉蛋白质的储存情况，而且不受炎症、容量状态的影响，是一种评估营养状态的敏感指标，在营养评估中应得到充分的重视，特别在追踪同一个患者营养状态随透析时间延长的动态变化中。

Handgrip Strength: A Nutritional Marker in Peritoneal Dialysis Patients

Objective To analyze the association between handgrip strength and nutrition in peritoneal dialysis patients. Methods A total of 235 continuous ambulatory peritoneal dialysis patients were enrolled. The relationships between handgrip strength and other parameters were analyzed. Results Handgrip strength was positively correlated with weight, serum albumin, lean body mass, and normalized protein-equivalent nitrogen appea- rance (P<0.05). Regression analysis showed that sex and age were significant independent predictors of handgrip strength (P<0.05). Conclusions Handgrip strength is a cost-effective marker of nutrition. It reflects the protein reserves in human muscle without being confounded by inflammation or the hydration status. Handgrip strength may be used as a regular nutrition-monitoring tool in dialysis patients.
