未破裂输卵管妊娠行药物保守疗法与手术保守疗法的比较

目的探讨治疗未破裂输卵管妊娠的药物保守疗法和手术保守疗法的优缺点。方法比较药物保守疗法（MTX法）和手术保守疗法（腹腔镜下患侧输卵管开窗术）在住院天数、治愈成功率、治疗成功后转经天数、血HCG值降至正常天数、输卵管肿块消失天数、治疗后输卵管疏通率、治疗后下腹痛率、治疗后妊娠率上的不同点。结果两种方法在转经时间、血HCG值降至正常天数上无明显差异；但在住院天数、治疗成功率、治疗后下腹痛率、治疗后输卵管通畅率、治疗后妊娠率上有显著差异。MTX疗法相比腹腔镜下患侧输卵管开窗术疗法住院时间长，治疗成功率低，治疗后下腹痛者多，肿块消失时间长，治疗后输卵管通畅率低、治疗后妊娠率低。结论腹腔镜保守手术治疗输卵管妊娠比药物治疗输卵管妊娠有更高的治疗成功率、患侧输卵管畅通率和妊娠率，且住院时间短、治疗后不适感少，有利于患者，值得推广和应用。

Comparison of the medicaments and laparoscopic operation on the therapy of tubal pregnancy without rupture

ObjectiveTo evaluate advantage and disadvantage of Medicaments(MTX) and Laparoscopic operation on the therapy of tuba pregnancy without rupture.MethodsTo compare the differentia of Methotrexate(MTX) and Laparoscopic operation therapy on the hospitalization days,the achievement ratio of treatment,menstruation days after therapy,the days of HCG droped in normal,the lump of oviduct absorption days,oviduct repatency rates after therapy,and bellyache rates after therapy,pregnancy rates after therapy.ResultsMTX and Laparoscopic operation treatment have no statistical difference on menstruation days after therapy and the days of HCG dropped in normal;but have a statistical difference on hospitalization days,the achievement ratio of treatment,bellyache rates after therapy,oviduct repatency rates after therapy,pregnancy rates after therapy.ConclusionLaparoscopic operation on the therapy of tubal pregnancy superior to Methotrexate(MTX) on the shortened hospitalization days,lower bellyache rates,and highly achievement ratio of treatment and pregnancy rates.
