听神经瘤术中肌电图应用的临床意义

摘    要:目的:探讨肌电图监测在听神经瘤术中的临床意义。方法:回顾性分析从2005年1月-2011年7月间完成的25例听神经瘤手术患者的临床资料,其中直接显微镜下切除肿瘤(非监测组)11例,显微镜下切除肿瘤过程中使用肌电图监测仪进行监测(监测组)14例。通过House—Braekmann评分标准评价两组患者术前、术后2周的面神经功能状态;同时比较两组面神经解剖保留率情况。结果:本组25例,肿瘤全切21例,全切率为84%(21/25),非监测组面神经保留率为54.54%(6/11),监测组为92.85%(13/14)。监测组高于非监测组,对两组患者术前、术后2周的面神经功能状态数据进行分析,两组术前面神经功能差异无统计学意义,术后2周面神经功能差异有统计学意义;结论:肌电图监测应用在听神经瘤切除手中可以明确面神经走行,有效保留面神经功能,具有重要的临床意义。

Abstract:Objective:To investigate the clinical neuroma. Method : From January 2005 to July 2011,25 signincance of electromyography monitoring in acoustic cases with completion of neurosurgieal acoustic neuroma surgery were retrospectively analysis, the clinical data were collected also. Of all the cases, removed the tumor under the microscope directly( non- monitoring group)ibr 11 eases, microscopic resection used in tumor surgery of facial nerve monitor nerve monitoring( monitoring group)tbr 14 cases. Facial nerve function status were evaluated by House - Braekmann Scoring before and after 2 weeks, and the retention rates of facial nerve anat- omy were compared at the same time. Results:Of all this 25 patients, total resection rate was 84% (21/25), retention rates of facial nerve anatomy in non - monitoring group and monitoring group was 54.54% ( 6/11 ) and 92.85% ( 13/14 ), respectively. Analysed tile data of facial nerve function inpatients in both groups before and after 2 weeks, there was no significant difference before surgery, but the difference was statistically significant after 2 weeks. Conclus;.ons:Eiectromyography monitoring applications in acoustic neuroma surgery can clear away the facial lines, preserve the facial nerve function effectively, which has important clinical significance.
