TCD检测妊娠高血压综合征患者脑血流变化的临床研究

目的:研究妊娠高血压综合征(简称妊高征)脑血流变化。方法:利用经颅超声多普勒技术(TCD),检测90例妊高征(观察组)、50例正常孕妇(对照组)的ACA(大脑前动脉)、MCA(大脑中动脉)、PCA(大脑后动脉)的血流频谱参数(V_(?)、V_d、V_m、PI)值结果,观察组的脑血流增快和减慢及其血管所占的百分比与对照组相比差异极显著(p＜0.01);观察组中重度与轻中度之间脑血流增快和减慢差异显著(p＜0.05);观察组中轻、中度以脑血流速增快居多,重度以脑血流减慢居多。结论:轻中度妊高脑血管痉挛程度轻,以代偿性加快为多,重度妊高征以失代偿减慢的多,早期行TCD检查,可发现脑血流异常,尽早治疗,对预防子痫及脑血管意外有重要意义。

Clinical research on the changes of the brain blood flow of the patients with pregnant hypertension syndrome

To study the brain blood flow in pregnant hyperfension syndrome Method:The transcraninal Doppler (TCD) was used to investigate the blood flow frequency spectrum parameters ( Vs, Vd, Vm, PI) of ACA ( anterior cerebral artemy) , MCA (meddle cerebral artery ) , PCA (posterior cerebral artery) in the 90 cases of pregnant hypertension syndrome patients (the observation group) and 50 cases of normal pregnant women (the control group) . Result: There was significant difference between the observation group and the control group in the increase and decrease of the blood flow velosities and the percentage of blood vessels( p < 0.01) ; In the observation group , there were significant differences between the severe patients and the mild and moderate patients on the increase and decrease of the brain blood flow ( p < 0.05) ; In the observation group, most of the mild and moderate patients indicated the increase of the brain blood flow while most of the severe patient indicated the decrease of the brain blood flow. Conclusion: The spasm of brain blood vessels of the mild and moderate patients were mild , for the blood flow mainly increased for the compensation, However, blood flow of the severe patients mainly decreased for decompensation. The early TCD examination could show the abnormal brain blood flow , and the early treatments is very important to prevent the eclampsia and the cerebral vascular accident.
