经腹羊膜腔灌注术治疗妊娠32周前合并羊水过少的临床研究

摘    要:目的探讨妊娠32周前发生羊水过少的孕妇进行经腹羊膜腔灌注术后的围生儿结局及安全性评估。方法对81例32周前发生羊水过少的孕妇按自愿原则分组进行经腹羊膜腔灌注术及传统的补液治疗,观察围生儿结局及产科并发症情况。结果 81例患者中,有11例在羊膜腔灌注后B超诊断胎儿畸形。灌注组的新生儿平均出生周数为(37.1±2.47)周,出生体重为(2738.95±385.90)g,均大于补液组(P<0.05);而早产、胎儿窘迫、新生儿畸形、新生儿黄疸、缺血缺氧性脑病、肺透明膜病、组织学绒毛膜羊膜炎发生率低于补液组,两组差异有统计学意义(P<0.05)。两组在分娩方式、宫内感染、胎膜早破等并发症的差异无统计学意义(P>0.05)。结论对孕32周前发生羊水过少的孕妇进行经腹羊膜腔灌注术能及时排除胎儿畸形,并有效改善围生儿预后,操作安全性高。
